摘要
目的 探讨多模态超声检查对备孕期女性子宫内膜容受性(ER)的评估价值。方法 选取我院备孕女性92例,其中41例备孕受孕失败(备孕时间> 6个月未受孕)作为观察组,51例备孕受孕成功(备孕时间≤6个月)作为对照组。比较两组一般资料、血清性激素[孕酮(P)、雌二醇(E2)、促黄体生成素(LH)、促卵泡生成素(FSH)]水平、子宫内膜厚度及子宫内膜厚度评分、内膜类型评分、内膜运动评分、内膜血流分布评分、内膜体积评分、内膜血管血流指数评分、多模态超声检查评分、子宫内膜血流参数[搏动指数(PI)、阻力指数(RI)、收缩峰速度与舒张末期速度比值(S/D)],分析多模态超声检查评分、子宫内膜血流参数预测备孕受孕结果的价值。结果 观察组子宫内膜厚度、子宫内膜厚度评分、内膜血管血流指数评分、多模态超声检查评分均低于对照组,PI、RI、S/D值均高于对照组(P <0. 05);多模态超声检查评分、PI、RI、S/D值联合预测备孕受孕失败的曲线下面积(AUC)分别为0. 936。结论 多模态超声检查可对备孕期女性ER进行全面、客观评估,多模态超声评检查分联合子宫内膜血流参数对预测备孕女性是否能成功受孕有一定参考价值。
Objective To investigate the value of multimodal ultrasound in the assessment of endometrial tolerance(ER)in women preparing for pregnancy.Methods Ninety-two pregnant women in our hospital were selected,of whom 41 with failed pregnancy preparation and conception(time to conception>6 months without conception)were used as the study group and 51 cases with successful pregnancy preparation and conception(time to conception≤6 months)were used as the control group.The general data,serum sex hormones[progesterone(P),estradiol(E2),luteinizing hormone(LH),follicle stimulating hormone(FSH)]levels,endometrial thickness and endometrial thickness score,endometrial type score,endometrial motility score,endometrial blood flow distribution score,endometrial volume score,endometrial vascular blood flow index score,multimodal ultrasound score,endometrial blood flow parameters[pulsatility index(PI),resistance index(RI),and peak systolic velocity to end-diastolic velocity ratio(S/D)],were compared the value of multimodal ultrasound scores,and endometrial blood flow parameters were analyzed to predict the outcome of pregnancy preparation and conception.Results Endometrial thickness,endometrial thickness score,endometrial vascular blood flow index score,and multimodal ultrasound score were lower in the study group than those in the control group,and PI,RI,and S/D values were higher than those in the control group(P<0.05);the area under the curve(AUC)of the combined multimodal ultrasound score,PI,RI,and S/D values for predicting conception failure in pregnancy preparation was 0.936,respectively.Conclusion Multimodal ultrasound can provide multimodal ultrasound scores combined with endometrial blood flow parameters that have some reference value in predicting the successful conception of women in pregnancy preparation.
