阿尔茨海默病和轻度认知障碍患者海马区MRI表现及其与认知功能的关系 被引量:7

Comparison of MRI Findings in Hippocampus of Patients with Alzheimer′s Disease and Mild Cognitive Impairment and Its Relationship with Cognitive Function

摘要 目的比较阿尔茨海默病(AD)与轻度认知障碍(MCI)患者海马区MRI的表现,探究其与认知功能的关系。方法回顾性分析西安市精神卫生中心2018年1月至2020年1月收取的35例AD患者与26例MCI患者的MRI影像学资料,观察两组患者海马区(海马高度、颞角宽度、海马体积、岛叶厚度、杏仁核体积)指标差异,采用Pearson相关性分析其与认知功能的关系。结果MMSE量表结果显示,AD组患者定向力、注意力与计算力、即刻记忆、短时记忆、语言能力和视空间技能、MMSE总分均低于MCI组(P均<0.05);AD组患者左右侧海马区海马高度、海马体积、杏仁核体积指标均低于MCI组,颞角宽度高于MCI组(P均<0.05);Pearson相关性结果显示,海马高度、颞角宽度、岛叶厚度、杏仁核体积与认知功能呈正相关(r=0.374、r=0.486、r=0.411、r=0.603,P均<0.05);认知功能与海马高度、海马体积、岛叶厚度、杏仁核体积呈正相关(r=0.667、r=0.715、r=0.491、r=0.721,P均<0.05)。结论应用MRI可以很好地评估AD与MCI海马区参数;使用MMSE量表对患者评价认知功能效果较好,海马区参数与认知功能有一定的相关性。 Objective Compare the MRI manifestations of hippocampus in patients with Alzheimer's disease(AD)and mild cognitive impairment(MCI)to explore their relationship with cognitive function.Methods Retrospectively analyze the MRI imaging data of 35 patients with AD and 26 patients with MCI in Xi'an Mental Health Center from January 2018 to January 2020,and observe the hippocampus(hippocampal height,temporal horn width,hippocampal volume,insula thickness of the two groups of patients,amygdala volume)index difference,using Pearson correlation to analyze its relationship with cognitive function.Results The results of the MMSE scale showed that the AD group's orientation,attention and calculation,immediate memory,short-term memory,language ability and visual spatial skills,and MMSE total scores were all lower than those of the MCI group(P all<0.05);the left and right hippocampus indexes of the AD group of hippocampus height,hippocampal volume,and amygdala volume were lower than those in the MCI group,and the width of the temporal horn was higher than that in the MCI group;Pearson correlation results showed that hippocampus height,temporal horn width,insula thickness,amygdala volume and cognitive function are significantly positively correlated with cognitive function(r=0.374,r=0.486,r=0.411,r=0.603,P all<0.05);cognitive function is significantly positively correlated with hippocampal height,hippocampal volume,insula thickness,and amygdala volume(r=0.667,r=0.715,r=0.491,r=0.721,P all<0.05).Conclusion MRI can be used to evaluate AD and MCI hippocampal parameters well;with cognitive function the use of MMSE scale has a good effect on evaluating patients'cognitive function,and hippocampal parameters have a certain correlation with cognitive function.
