胃充盈超声检查评估甲状腺功能亢进患者胃动力功能的临床价值

Clinical value of gastric filling ultrasonography in the evaluation of gastric motility in patients with hyperthyroidism

目的应用胃充盈超声检查评估甲状腺功能亢进患者的胃动力功能和临床价值,并探讨甲亢患者血清促甲状腺激素(TSH)水平与胃动力功能之间的相关性。方法选取皖南医学院第一附属医院2021年10月—2022年1月期间收治的甲状腺功能亢进患者30例作为研究对象,同期健康志愿者30例作为对照组,测定甲状腺功能亢进患者血清促甲状腺激素(TSH),用胃充盈超声分别测量两组受试者饮用后即刻的最大胃窦舒张区(S)、胃窦收缩幅度(ACA)、胃窦收缩频率(ACF)及胃窦运动指数(MI),分析甲状腺功能亢进患者的胃动力功能,并用M型超声观测胃窦收缩变化率。结果①两组受试者ACA比较,差异无统计学意义(P>0.05);②两组受试者S、ACF及MI与对照组相比,差异有统计学意义(P<0.05);③甲状腺功能亢进患者的血清TSH水平与ACF及MI呈负相关(P<0.05);④M型超声测量甲状腺功能亢进患者胃窦收缩变化差异有统计学意义(P<0.001)。结论甲状腺功能亢进可导致胃动力功能加快,同时,甲状腺功能亢进患者的血清促甲状腺激素(TSH)水平与胃窦蠕动呈良好的负相关,并且,M型超声测量甲状腺功能亢进患者胃窦收缩变化具有一定的临床参考价值。

Objective To evaluate gastric motility and clinical value in patients with hyperthyroidism with gastric filling ultrasonography,and to investigate the correlation between the serum thyroid-stimulating hormone(TSH)levels and gastric motility in patients with hyperthyroidism.Methods A total of 30 patients with hyperthyroidism admitted to the First Affiliated Hospital of Wannan Medical College from October 2021 to January 2022 were selected as the study subjects,and 30 healthy volunteers were enrolled as the control group.The serum TSH were measured in patients with hyperthyroidism,and the maximal diastolic area of gastric antrum,gastric antral contraction amplitude(ACA),gastric antral contraction frequency(ACF)and gastric antral motility index(MI)of two groups were measured immediately with gastric filling ultrasonography after drinking related agents so as to analyze the gastric motility of hyperthyroidism patients.The rate of change of gastric antral contraction was measured by M-mode ultrasound in patients hyperthyroidism.Results①ACA had no statistical difference between the study group and the control group(P>0.05);②There were statistical differences in the maximal diastolic area of gastric antrum,ACF and MI between two groups(P<0.05);③The serum TSH levels were negatively correlated with ACF and MI in patients with hyperthyroidism(P<0.05);④The rate of change of gastric antral contraction in patients with hyperthyroidism measured by M-mode ultrasound had statistical difference between the study group and the control group(P<0.001).Conclusion Hyperthyroidism can accelerate gastric motility,and the serum TSH levels in patients with hyperthyroidism have a significant negative correlation with gastric antral peristalsis.The M-mode ultrasound has a certain clinical value to measure the change of gastric antral contraction in patients with hyperthyroidism.
