腹腔镜联合胆道镜胆总管探查与开腹手术对胆总管结石患者的治疗效果及对患者生活质量的影响

The treatment efficacy of laparoscope combined with choledochoscope exploration of common bile duct and laparotomy on patients with common bile duct stones and their influence on quality of life of the patients

目的分析腹腔镜联合胆道镜胆总管探查与开腹手术对胆总管结石患者的治疗效果及对患者生活质量的影响。方法选择2019年1月—2021年1月在本院应用两镜联合技术治疗的86例胆总管结石患者作为研究对象,随机数表法分为常规组和研究组两组,每组各43例。研究组采用腹腔镜联合胆道镜进行治疗,常规组采用传统开腹手术进行治疗。比较两组患者的术后恢复情况、应激指标、免疫功能、生活质量、IgG与PCT水平、并发症的总发生率。结果研究组临床恢复时间较低(P<0.05)。术后研究组EP、CO、IL-6、CRP、IgG水平较低(P<0.05);NK、PCT水平、免疫功能、生活质量评分较高(P<0.05)。研究组并发症发生率明显降低(P<0.05)。结论腹腔镜联合胆道镜对治疗胆总管结石患者临床效果较好,在一定程度上改善患者机体应激状态,且对患者体内免疫功能影响较小,术后恢复较快,并发症少,提高患者的生活质量,值得临床应用与推广。

Objective To analyze the therapy effect of laparoscope combined with choledochoscope exploration of common bile duct and laparotomy on the treatment of common bile duct stones and the impact on the quality of life of patients.Methods From January 2019 to January 2021,86 patients who suffered from common bile duct stones treated with a combination technology of laparoscope and choledochoscope were enrolled as study subjects.They were randomly divided into a routine group and a study group using the random number table method.The treatment method used in the study group was laparoscope combined with choledochoscope,and the conventional group was treated with traditional open surgery.The postoperative recovery,stress indexes,immune function,quality of life,IgG and PCT levels,and the total incidence of complications were compared between the two groups.Results The clinical recovery time of the study group was significantly lower than that of the conventional group(P<0.05).The postoperativelevels of EP,CO,IL-6,CRP,and IgG in the study group were significantly lower than those in the conventional group(P<0.05).The scores of NK and PCT levels,immune function,and quality of life were significantly higher than those in the conventional group(P<0.05).The complication rate in the study group was significantly decreased(P<0.05).Conclusions Laparoscope combined with choledochoscope has a good clinical effect in treating patients with choledocholithiasis.It could improve the stress state of patients to a certain extent and has little impact on the immune function of patients,with faster postoperative recovery and fewer complications,and improve the quality of life of patients.It is worthy of clinical application and promotion.
