叙事护理对肿瘤科护士职业倦怠、护士满意度干预效果分析 被引量:2

Analysis on the intervention effect of narrative nursing on job burnout and satisfaction of nurses in the oncology department

摘要 目的探讨叙事护理对肿瘤科护士职业倦怠、护士满意度的干预效果。方法选择2021年1―12月就职于本院肿瘤科的70名护士作为研究对象,采用随机数表法分为观察组和对照组两组,每组各35名。对照组使用常规护理培训,观察组在此基础上开展叙事护理培训并开展叙事护理实践,比较两组护士的职业倦怠、护士满意度、护士情绪与自我管理能力及共情能力。结果干预后,两组护士情感枯竭、去个性化和成就感缺失评分均降低,且观察组以上得分均低于对照组,差异均有统计学意义(均P<0.05)。干预后,两组护士积极情绪量表(PA)和正性信息注意量表(API)评分均升高,消极情绪量表(NA)、负性信息注意量表(ANI)评分降低,且观察组干预后以上得分均低于对照组,差异均有统计学意义(均P<0.05)。干预后,两组护士自我减压、自我决策及正性态度评分均升高,且观察组干预后以上得分均高于对照组,差异均有统计学意义(均P<0.05)。干预后,两组护士汉密尔顿焦虑量表(HAMA)评分和汉密尔顿抑郁量表(HAMD)评分均降低,且观察组干预后以上得分均低于对照组,差异均有统计学意义(均P<0.05)。干预后,两组护士观点采择、情感护理及换位思考评分均升高,且观察组干预后以上得分均高于对照组,差异均有统计学意义(均P<0.05)。观察组护士的护士满意度(激励的绩效制度,困难可得到帮助,职业提升、职业安全感、工作量适度)高于对照组,差异均有统计学意义(均P<0.05)。结论叙事护理模式可有效减轻肿瘤科护士的职业倦怠感,提高护理人员自我管理能力、共情能力,可作为一种有效的干预手段在临床推广。 Objective To study the intervention effect of narrative nursing on job burnout and satisfaction of nurses in the oncology department.Methods A total of 70 nurses who worked in the oncology department,Shenzhen Hospital,Beijing University of Traditional Chinese Medicine,from January to December 2021,were selected as study subjects.They were divided into an observation group(n=35)and a control group(n=35)according to random number table method.The control group received routine nursing training,and the observation group received narrative nursing training and practice on this basis.The job burnout,nurses’satisfaction,nurses’emotion,self-management ability,and empathy ability of the two groups were compared.Results After intervention,the scores of emotional exhaustion,depersonalization,and lack of personal achievement in the two groups were significantly decreased,and the scores in the observation group were significantly lower than those in the control group(P<0.05).The scores of positive affect scale(PA)and attention to positive inventory scale(API)were significantly increased,while the scores of negative affect scale(NA)and attention to negative inventoryscale(ANI)were decreased,and the above scores in the observation group were significantly lower than those in the control group(P<0.05).After the intervention,the scores of self-decompression,self-decision-making and positive attitude in the two groups were increased,and those in the observation group were significantly higher than those in the control group(P<0.05).After the intervention,the score of HAMA and HAMD scales of the two groups decreased significantly,and the above scores in the observation group were significantly lower than those of the control group(P<0.05).After the intervention,the scores of viewpoint selection,emotional nursing,and transposition thinking in the two groups increased significantly,and the above scores of the observation group after intervention were significantly higher than those in the control group(P<0.05).Nurses in the observation group were more satisfied with the incentive performance system,solved difficulties,career advancement,job security,and moderate workload than the control group(P<0.05).Conclusions Narrative nursing model could significantly relieve job burnout among nurses in the oncology department and improve the self-management ability and empathy ability of nurses.It could be popularized in clinics as an effective intervention method.
