奥曲肽联合凝血酶治疗急性消化道出血的临床效果

Clinical effect of octreotide combined with thrombin in the treatment of acute gastrointestinal bleeding

目的探讨奥曲肽联合凝血酶治疗急性消化道出血的临床效果。方法选取2019年4月至2021年4月我院诊治的82例急性消化道出血患者作为研究对象,根据随机法将其分为单纯组(41例,奥曲肽)与联合组(41例,奥曲肽联合凝血酶)。比较两组的治疗效果。结果24 h时,联合组的止血率高于单纯组(P<0.05);72 h时,联合组的止血率低于单纯组(P<0.05)。治疗3 d后,联合组的丙二醛、脂质过氧化氢水平低于单纯组,谷胱甘肽过氧化物酶、超氧化歧化酶水平高于单纯组,差异具有统计学意义(P<0.05)。治疗3 d后,联合组的谷草转氨酶(AST)、谷丙转氨酶(ALT)、总胆红素(TBil)水平低于单纯组,差异具有统计学意义(P<0.05)。结论奥曲肽联合凝血酶治疗急性消化道出血的效果显著,有利于改善氧化应激、肝功能指标,值得临床应用。

Objective To investigate the clinical effect of octreotide combined with thrombin in the treatment of acute gastrointestinal bleeding.Methods A total of 82 patients with acute gastrointestinal bleeding diagnosed and treated in our hospital from April 2019 to April 2021 were selected as the research objects.According to the random method,the patients were divided into simple group(41 cases,octreotide)and combined group(41 cases,octreotide combined with thrombin).The therapeutic effects of the two groups were compared.Results At 24 h,the hemostasis rate of the combined group was higher than that of the simple group(P<0.05);at 72 h,the hemostasis rate of the combined group was lower than that of the simple group(P<0.05).After 3 d of treatment,the levels of malondialdehyde and lipid hydrogen peroxide in the combined group were lower than those in the simple group,the levels of glutathione peroxidase and superoxide dismutase were higher than those in the simple group,and the differences were statistically significant(P<0.05).After 3 d of treatment,the levels of aspartate transaminase(AST),alanine aminotransferase(ALT)and total bilirubin(TBil)in the combined group were lower than those in the simple group,and the differences were statistically significant(P<0.05).Conclusion Octreotide combined with thrombin in the treatment of acute gastrointestinal bleeding effect has a significant effect,and is conducive to improving oxidative stress and liver function indicators,which is worthy of clinical application.
