香砂理中汤配合四联疗法治疗脾胃虚弱型Hp相关性溃疡的有效性及安全性

Efficacy and safety of Xiangsha Lizhong decoction combined with quadruple therapy in the treatment of Hp related ulcer with spleen and stomach deficiency type

目的探讨香砂理中汤配合四联疗法治疗脾胃虚弱型幽门螺杆菌(Hp)相关性溃疡的有效性及安全性,以为临床治疗方案的制定提供参考。方法选取2019年9月至2021年3月我院收治的60例脾胃虚弱型Hp相关性溃疡患者作为研究对象,按照治疗方案不同将其分为单纯组和联合组,各30例。单纯组采用四联疗法治疗,联合组在单纯组基础上联合香砂理中汤治疗。比较两组的治疗效果。结果联合组的Hp根除成功率高于单纯组,差异具有统计学意义(P<0.05)。联合组的不良反应总发生率低于单纯组,差异具有统计学意义(P<0.05)。联合组的腹痛腹胀、反酸、嗳气、烧心消除时间及溃疡完全愈合时间短于单纯组,差异具有统计学意义(P<0.05)。治疗前,两组的白蛋白(ALB)、血红蛋白(Hb)、总蛋白(TP)水平比较,差异无统计学意义(P>0.05);治疗后,两组的ALB、Hb、TP水平均升高,且联合组高于单纯组,差异具有统计学意义(P<0.05)。治疗前,两组的舌质胖大、畏寒肢冷、胃脘隐痛、泛吐酸水积分比较,差异无统计学意义(P>0.05);治疗后,两组的舌质胖大、畏寒肢冷、胃脘隐痛、泛吐酸水积分均降低,且联合组低于单纯组,差异具有统计学意义(P<0.05)。结论香砂理中汤配合四联疗法治疗脾胃虚弱型Hp相关性溃疡可减轻疾病症状,改善营养状态,且安全系数高,值得临床推广和应用。

Objective To explore the efficacy and safety of Xiangsha Lizhong decoction and quadruple therapy in the treatment of Helicobacter pylori(Hp)related ulcer with spleen and stomach deficiency type,so as to provide reference for the formulation of clinical treatment scheme.Methods A total of 60 patients with Hp related ulcer with spleen and stomach deficiency type admitted in our hospital from September 2019 to March 2021 were selected as the research objects,and the patients were divided into simple group and combined group according to different treatment schemes,with 30 cases in each group.The simple group was treated with quadruple therapy,and the combined group was treated with Xiangsha Lizhong decoction on the basis of the simple group.The therapeutic effect of the two groups were compared.Results The success rate of Hp eradication in the combined group was higher than that in the simple group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the combined group was lower than that in the simple group,and the difference was statistically significant(P<0.05).The elimination time of abdominal pain and abdominal distension,acid regurgitation,belching and heartburn and complete healing time of ulcer in the combined group were shorter than those in the simple group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of albumin(ALB),hemoglobin(Hb)and total protein(TP)between two groups(P>0.05);after treatment,the levels of ALB,Hb and TP in the two groups increased,and those in the combined group were higher than the simple group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in the scores of tongue fat big,chilliness and cold limbs,stomachache and acid regurgitation between the two groups(P>0.05);after treatment,the scores of tongue fat big,chilliness and cold limbs,stomachache and acid regurgitation in the two groups decreased,and those in the combined group were lower than the simple group,the differences were statistically significant(P<0.05).Conclusion Xiangsha Lizhong decoction combined with quadruple therapy in the treatment of Hp related ulcer with spleen and stomach deficiency type can reduce the disease symptom,improve the nutritional status,and has a high safety factor.It is worthy of clinical promotion and application.
