希望水平与疾病应对方式在乳腺癌患者社会支持和生命质量间的链式中介作用

Chain Mediating Effect of Hope Level and Disease Coping Style on Social Support and Quality of Life in Breast Cancer Patients

目的:探讨希望水平与疾病应对方式在乳腺癌患者社会支持和生命质量间的链式中介效应,为提升该人群生命质量提供理论依据。方法:选取新疆某三级甲等肿瘤专科医院290例乳腺癌患者作为研究对象,采用Herth希望指数量表(Herth hope index scale,HHI)、医学应对方式问卷(medical coping style questionnaire,MCMQ)、社会支持评定量表(social support rating scale,SSRS)、癌症患者生命质量核心量表(cancer patient quality of life core scale,QLQ-30)开展调查。采用SPSS及Bootstrap法构建链式中介模型并检验。结果:乳腺癌患者HHI得分为(27.93±4.28)分,SSRS得分为(32.69±7.51)分,MCMQ得分为(45.39±5.72)分,QLQ-30得分为(56.27±15.84)分。乳腺癌患者社会支持与希望水平、面对应对方式、生命质量间呈两两正相关(均P<0.01),与屈服应对方式呈负相关(P<0.01)。经中介效应检验,希望水平与面对应对方式在社会支持对生命质量的影响中起链式中介作用。结论:社会支持既能直接预测乳腺癌患者生命质量,又可以通过希望水平与面对应对方式的链式中介作用间接预测其生命质量。

Objective:To explore the chain mediating effect of hope level and disease coping style on social support and quality of life of breast cancer patients,to provide a theoretical basis for improving the quality of life of the population.Methods:A total of 290 breast cancer patients in a grade A tertiary cancer hospital in Xinjiang were selected as subjects.Herth hope index scale(HHI),medical coping style questionnaire(MCMQ),social support rating scale(SSRS)and cancer patient quality of life core scale(QLQ-30)were used to investigate.The chain mediation model was constructed by using SPSS macro program process plug-in and Bootstrap method.Results:The scores of HHI,SSRS,MCMQ and QLQ-30 were(27.93±4.28),(32.69±7.51),(45.39±5.72)and(56.27±15.84)respectively.There was positive correlation between social support and hope level,coping style and quality of life(all P<0.01),and negative correlation between social support and coping style(P<0.01).Through the mediating effect test,hope level and coping style play a chain mediating role in the influence of social support on quality of life.Conclusion:Social support can not only directly predict the quality of life of breast cancer patients,but also indirectly predict the quality of life of breast cancer patients through the chain mediating effect of hope level and coping style.
