柱状水囊前列腺扩开术对良性前列腺增生患者国际勃起功能指数的影响

The influence of columnar balloon dilatation of prostate on the international index of erectile function in patients with benign prostatic hyperplasia

目的分析柱状水囊前列腺扩开术对良性前列腺增生(BPH)患者国际勃起功能指数(IIEF)的影响。方法选取2019年1月至2021年1月首都医科大学附属北京友谊医院收治的120例BPH患者作为研究对象。根据不同治疗术式分为三组,每组40例。对照组采用经尿道前列腺电切术(TURP)治疗,剜除组采用经尿道前列腺钬激光剜除术(HoLEP)治疗,扩开组采用经尿道柱状水囊前列腺扩开术(TUCBDP)治疗。比较三组的围手术期指标、IIEF评分、国际前列腺症状评分法(IPSS)评分。结果扩开组与剜除组手术时间、血红蛋白下降量、膀胱持续冲洗时间、导尿管留置时间比较,差异无统计学意义(P>0.05),但手术时间均长于对照组,血红蛋白下降量均低于对照组,膀胱持续冲洗时间、导尿管留置时间均短于对照组,差异具有统计学意义(P<0.05);三组住院时间比较,差异无统计学意义(P>0.05);术前三组IIEF评分、IPSS评分比较,差异无统计学意义(P>0.05),术后3个月三组均较术前改善且三组间IIEF评分比较,差异具有统计学意义(P<0.05),三组间IPSS评分比较,差异无统计学意义(P>0.05);三组间并发症发生率比较,差异无统计学意义(P>0.05)。结论BPH患者采用TUCBDP治疗有助于提升患者的IIEF评分,且近期疗效较好。

Objective To analyze the effect of columnar balloon dilatation of prostate on the international index of erectile function(IIEF)in patients with benign prostatic hyperplasia(BPH).Methods A total of 120 patients with BPH who were treated in Beijing Friendship Hospital,Capital Medical University from January 2019 to January 2021 were selected as the research objects,and divided into the following three groups according to the treatment methods,with 40 cases in each group.The control group was treated with transurethral resection of the prostate(TURP),the enucleation group was treated with holmium laser enucleation of the prostate(HoLEP)and the dilation group was treated with transurethral columnar balloon dilatation of prostate(TUCBDP).The perioperative indexes,IIEF scores and international prostate symptom score(IPSS)of the three groups were compared.Results There was no significant difference in operation time,hemoglobin decrease,bladder continuous irrigation time and catheter indwelling time between the enucleation group and the dilation group(P>0.05),but the operation time was longer than that in the control group,the decrease in hemoglobin was lower than that in the control group,the bladder continuous irrigation time and catheter indwelling time were shorter than those in the control group(P<0.05).There was no significant difference in the length of hospital stay among the three groups(P>0.05).There was no significant difference in the IIEF score and IPSS score among the three groups before operation(P>0.05).There was significant difference in the IIEF score among the three groups at 3 months after operation(P<0.05).There was no significant difference in the IPSS score among the three groups at 3 months after operation(P>0.05).There was no significant difference in the incidence of complications among the three groups(P>0.05).Conclusions TUCBDP is more helpful to improve the IIEF score of BPH patients,and the short-term curative effect is better.
