良性前列腺增生患者前列腺剜除术后发生尿失禁的相关因素 被引量:5

摘要 目的采用Logistic回归分析良性前列腺增生(BPH)患者前列腺剜除术后发生尿失禁的影响因素。方法选取2020年5月至2021年5月北京市顺义医院诊治的49例接受前列腺剜除术后发生尿失禁的BPH患者作为研究对象,纳入发生组;将同期诊治的49例接受前列腺剜除术后未发生尿失禁的BPH患者纳入未发生组。分析两组的基线资料、实验室相关指标等,分析其发生尿失禁的影响因素。结果发生组合并糖尿病、合并高血压占比高于未发生组,术前膜性尿道长度(MUL)短于未发生组,术前前列腺体积高于未发生组(P<0.05);Logistic回归分析显示,糖尿病控制不稳定、高血压控制不稳定、术前前列腺体积大是BPH患者前列腺剜除术后发生尿失禁的危险因素(OR>1,P<0.05),术前MUL长是其保护因素(OR<1,P<0.05)。结论BPH患者前列腺剜除术后发生尿失禁可能受到糖尿病、高血压、术前MUL、术前前列腺体积的影响。 Objective To analyze the influencing factors of urinary incontinence after enucleation of prostate in patients with benign prostatic hyperplasia(BPH)by Logistic regression.Methods A total of 49 BPH patients with urinary incontinence after enucleation of prostate in Beijing Shunyi Hospital from May 2020 to May 2021 were selected as the research objects and included in the occurrence group.A total of 49 BPH patients without urinary incontinence after enucleation of the prostate in same period were included in the non-occurence group.The baseline data and laboratory related indicators of the two groups were analyzed,and the influencing factors of urinary incontinence were analyzed.Results The proportion of diabetes mellitus and hypertension in the occurrence group was higher than that in the non-occurrence group,the preoperative membranous urethral length(MUL)was shorter than that in the non-occurrence group,and the preoperative prostate volume was higher than that in the non-occurrence group(P<0.05).Logistic regression analysis showed that unstable control of diabetes,unstable control of hypertension and large preoperative prostate volume were risk factors for urinary incontinence in patients with BPH after enucleation of prostate(OR>1,P<0.05),and preoperative MUL longer was the protective factor(OR<1,P<0.05).Conclusions Urinary incontinence after enucleation of prostate in BPH patients may be affected by diabetes mellitus,hypertension,preoperative MUL and preoperative prostate volume.
