不同舌苔类型MPP患儿临床特征及相关炎症因子表达探讨

Clinical Characteristics and Expression of Related Inflammatory Factors in Children with Different Tongue Coating Types of Mycoplasma Pneumoniae Pneumonia

目的探讨不同舌苔类型儿童肺炎支原体肺炎(MPP)的临床特征、实验室检测结果、影像学表现及相关炎症因子的表达。方法选取2018年8月—2019年3月因支原体肺炎呼吸科住院的患儿,按照舌苔类型主要分型分为舌苔黄厚型与舌苔白腻型,收集并分析两组患儿的临床资料。结果舌苔黄厚型MPP患儿的年龄较舌苔白腻型MPP患儿更大,分别为4.5(2.7~7.0)岁,3.25(1.0~5.4)岁;舌苔黄厚组热程为8(6.5~10)d,舌苔白腻组为5(2~7)d;淋巴细胞亚群中,舌苔黄厚组CD8+细胞比例(27.1%±8.0%)高于舌苔白腻组(22.7%±7.7%);舌苔黄厚组有8例(21.6%)患儿发生胸腔积液,舌苔白腻组仅1例(2.9%)出现;舌苔黄厚组外周血中GM-CSF及sB7-H3的浓度较舌苔白腻组更高,差异具有统计学意义(P<0.05)。结论不同舌苔类型的支原体肺炎患儿具有不同的临床特点,舌苔黄厚型的患儿偏年长儿,炎症反应更剧烈,临床症状更重。

Objective To investigate the clinical features,laboratory results and imaging features of mycoplasma pneumoniae pneumonia(MPP)in children with different tongue coating types.Methods The clinical data of the children who was hospitalized in respiratory department of MPP from August 2018 to March 2019 were collected and analyzed.The children were divided into yellow thickness type and white greasy type according to the type of tongue coating.Results The age of MPP with yellow thickness of tongue coating was older than that of MPP with white tongue coating,which was 4.5(2.7-7.0)years and 3.25(1.0-5.4)years,respectively.The hot range of yellow thickness group was 8(6.5-10)days,and that of white greasy group was 5(2-7)days.The proportion of CD8+cells in tongue coating yellow thick group(27.1%±8.0%)was higher than that in tongue coating white greasy group(22.7%±7.7%),pleural effusions occurred in 8 cases(21.6%)in the yellow and thick tongue coating group,and only 1 case(2.9%)in the tongue coating white greasy group(P<0.05).The concentrations of GM-CSF and sB7-H3 in peripheral blood of yellow and thick tongue coating group were higher than those of tongue coating white greasy group(P<0.05),and the difference was statistically significant.Conclusion Children with MPP with different tongue coating types have different clinical characteristics.Children with yellow thick type of tongue coating are older,the inflammatory reaction is more intense,and the clinical symptoms are more severe.
