SNAP序列在颈动脉夹层的应用

摘要 目的探讨与常规黑血(BB)技术血管成像序列相比较,非对比增强血管和斑块内出血同时成像(SNAP)序列对颈动脉夹层疾病诊断的可行性。方法对36例怀疑为颈动脉夹层的患者行颅颈动脉高分辨率MRI检查,扫描序列包括SNAP、T1WI-BB、T2WI-BB及容积采集各向同性快速自旋回波质子加权成像(PDWI-VISTA)。在诊断颈动脉夹层过程中,分析与常规BB序列相比较,SNAP序列对颈动脉夹层患者诊断的灵敏度和特异度以及SNAP序列与常规BB序列诊断的一致性。结果(1)SNAP序列诊断颈动脉夹层的灵敏度为76.4%,特异度为96.4%。(2)SNAP序列与常规BB序列诊断颈动脉夹层有较好的一致性(Cohen's k=0.755,P<0.05)。结论SNAP序列成像有可能会成为筛查高度怀疑颈动脉夹层患者的首选序列。 Objective To investigate the feasibility of non-contrast-enhanced simultaneous angiography of vascular and intraplaque hemorrhage(SNAP)sequences in the diagnosis of carotid artery dissection compared with conventional black blood(BB)technique angiography sequences.Methods Thirty-six patients with suspected carotid artery dissection underwent the high magnetic resonance imaging of the cranio-cervical artery using sequences including SNAP,T1WI-BB,T2WI-BB and proton density weighted imaging-volume isotropic turbospin-echo acquisition(PDWI-VISTA).In the diagnosis of carotid artery dissection,the sensitivity and specificity of SNAP sequence for the diagnosis of patients with carotid artery dissection and the consistency between SNAP sequence and conventional BB sequence were analyzed compared with conventional BB sequence.Results During the diagnosis of carotid artery dissection,the SNAP sequence was found to provide good performance in diagnosis of carotid artery dissection(sensitivity 76.4%,specificity 96.4%),and good agreement was found between SNAP and conventional sequences(Cohen'k=0.755,P<0.05).Conclusion SNAP sequence imaging may be the preferred sequence for screening patients with highly suspected carotid artery dissection.
