液压输送式椎体成形术在骨质疏松性脊柱骨折患者中的应用效果及对结构参数的影响

Application Effect of Hydraulic Conveying Vertebroplasty in Patients with Osteoporotic Spinal Fracture and Influence on Structure Parameters

目的:探究液压输送式椎体成形术在骨质疏松性脊柱骨折患者中的应用效果及对结构参数的影响。方法:选择2020年6月-2021年12月在厦门市中医院进行椎体成形术治疗的100例骨质疏松性脊柱骨折患者,根据随机数字表法将其分为对照组和观察组,各50例。对照组进行推杆式骨水泥输送,观察组进行液压式骨水泥输送。比较两组的骨水泥弥散情况、不良情况发生率、手术前后的脊椎功能[Oswesty功能障碍指数(ODI)评分结果]、疼痛程度[视觉模拟评分法(VAS)评分结果]及结构参数(Cobb角、椎体压缩比及椎体前缘高度)。结果:观察组的骨水泥弥散情况显著优于对照组,不良情况发生率显著低于对照组,差异均有统计学意义(P<0.05)。术前两组的ODI评估结果、VAS评分及结构参数比较,差异无统计学意义(P>0.05);术后1 d、3个月及6个月,观察组的ODI评分、VAS评分及结构参数均显著优于术前,且观察组均显著优于对照组,差异均有统计学意义(P<0.05)。结论:液压输送式椎体成形术在骨质疏松性脊柱骨折患者中的应用效果相对更好,可显著改善结构参数及控制疼痛感,在骨质疏松性脊柱骨折患者中的应用价值相对更高。

Objective:To investigate the application effect of hydraulic conveying vertebroplasty in patients with osteoporotic spinal fracture and influence on structure parameters.Method:A total of 100 patients with osteoporotic spinal fracture treated with vertebroplasty in Xiamen Hospital of Traditional Chinese Medicine from June 2020 to December 2021 were selected and divided into control group and observation group according to the method of random number table,with 50 cases in each group.The control group was treated with pusher bone cement delivery,the observation group was treated with hydraulic bone cement delivery.Then the bone cement dispersion situation,total adverse events rates,spinal function[Oswesty dysfunction index (ODI) score result],pain degree[visual analogue scale (VAS) score result]and structure parameters (Cobb angle,compression ratio of vertebral body and height of leading edge of vertebral body) before and after the operation in the two groups were compared.Result:The bone cement dispersion situation in the observation group was significantly better than that in the control group,the total adverse events rate was significantly lower than that in the control group,the differences were statistically significant (P<0.05).Before operation,the ODI evaluation results,VAS score results and structure parameters in the two groups were compared,the differences were not statistically significant (P>0.05).At 1 day,3 and 6 months after operation,the ODI evaluation results,VAS score and structure parameters in observation group were all significantly better than those before the operation,and the observation group were all significantly better than those in the control group,the differences were statistically significant (P<0.05).Conclusion:The application effect of hydraulic conveying vertebroplasty in patients with osteoporotic spinal fracture is relatively better,and it can significantly improve the structure parameters and control the pain,so its application value in the patients with osteoporotic spinal fracture is relatively higher.
