IFN-γ、IL-2联合检测在结核病中的诊断价值

Diagnostic Value of Combined Detection of IFN-γ, IL-2 in Tuberculosis

目的:探讨结核特异性细胞因子干扰素-γ(IFN-γ)、白介素-2(IL-2)及联合检测在结核病中的诊断价值。方法:选取2021年5-7月广西壮族自治区胸科医院收治的462例疑似结核病患者作为研究对象。按照2018年最新实施的肺结核诊断标准(WS 288-2017),其中确诊结核病232例,纳入结核组,非结核病230例纳入非结核组。两组均进行结核特异性细胞因子IFN-γ和IL-2检测。比较两组结核特异性细胞因子IFN-γ和IL-2的表达水平。分析IFN-γ、IL-2及联合检测对结核病的诊断效能。结果:结核组IFN-γ和IL-2表达水平均明显高于非结核组(P<0.05)。联合检测AUC值明显高于IFN-γ、IL-2单独检测的AUC值(P<0.05)。联合检测的Youden指数最高为0.750,敏感度和特异度分别为89.3%和85.7%。结论:结核病患者的IFN-γ和IL-2明显升高,IFN-γ和IL-2对结核病具有较好的诊断效能,且二者联合检测的诊断价值更高。

Objective:To investigate the diagnostic value of tuberculosis specific cytokine interferon-γ (IFN-γ),interleukin-2 (IL-2) and their combined detection in tuberculosis.Method:A total of 462 suspected tuberculosis patients admitted to Chest Hospital of Guangxi Zhuang Autonomous Region from May to July 2021 were selected as the study subjects.According to the newest implemented diagnostic criteria for pulmonary tuberculosis (WS 288-2017) in 2018,among them,232 cases with tuberculosis were confirmed and included in the tuberculosis group,and 230 cases with non tuberculosis were included in the non tuberculosis group.Tuberculosis specific cytokine IFN-γ,IL-2 were detected in both groups.The expression levels of tuberculosis specific cytokine IFN-γ,IL-2 were compared between the two groups.The diagnostic efficacy of IFN-γ,IL-2 and their combined detection for tuberculosis was analyze.Result:The expression levels of IFN-γ and IL-2 in tuberculosis group were significantly higher than those in non tuberculosis group (P<0.05).The AUC value of combined detection was significantly higher than AUC value of IFN-γ and IL-2 alone detection (P<0.05).The highest Youden index was 0.750,the sensitivity and specificity were 89.3% and 85.7% respectively,and the accuracy was 87.5% in combined detection.Conclusion:The levels of IFN-γ and IL-2 in patients with tuberculosis are significantly increased.IFN-γ and IL-2 have better diagnostic efficacy for tuberculosis,and their combined detection has higher diagnostic value.
