常规康复治疗、淋巴引流联合开玄方对下肢关节置换术后肿胀疗效观察

Curative Effect of Routine Rehabilitation,Lymphatic Drainage Combine with Kaixuan Prescription on Postoperative Swelling of Lower Extremity Joint Arthroplasty Surgery

目的:观察常规康复治疗、淋巴引流联合开玄方对下肢关节置换术后肿胀的临床疗效。方法:选择2020年1-12月在漳州市中医院骨伤科住院行下肢关节置换术后肿胀患者90例,按照随机数字表法分为对照组、淋巴组和试验组,各30例。对照组给予常规康复治疗,淋巴组在常规康复治疗基础上给予淋巴引流,试验组在常规康治疗复、淋巴引流基础上口服开玄方,1次/d,疗程均为1周。于术后1、3、7 d观察三组肿胀度、疼痛评分,于术后1、3、7、14 d观察三组关节功能评分。结果:三组术后1 d肿胀度、VAS评分、关节功能评分比较,差异均无统计学意义(P>0.05);三组术后3、7 d肿胀度、VAS评分及术后3、7、14 d关节功能评分比较,差异均有统计学意义(P<0.05);试验组、淋巴组术后3、7 d肿胀度、VAS评分均低于对照组,且试验组术后3、7 d肿胀度及术后3 d的VAS评分均低于淋巴组,差异均有统计学意义(P<0.05);试验组、淋巴组术后3、7、14 d关节功能评分均高于对照组,且试验组术后3、7、14 d关节功能评分均高于淋巴组,差异均有统计学意义(P<0.05)。结论:常规康复治疗、淋巴引流联合开玄方对改善下肢关节置换术后肿胀具有显著疗效。

Objective:To explore the clinical effect of routine rehabilitation,lymphatic drainage combine with Kaixuan Prescription on postoperative swelling of lower extremity joint arthroplasty surgery.Method:Ninety patients with postoperative lower extremity joint arthroplasty surgery who treated in Orthopedics Department of Zhangzhou Hospital of Traditional Chinese Medicine from January to December 2020 were enrolled in this study,they were randomly divided into the control group,lymph group and observational group,30 cases in each group.The control group was given routine rehabilitation therapy,the lymph group was given lymphatic drainage on the routine rehabilitation therapy,the observational group was given Kaixuan Prescription on the basis of routine rehabilitation therapy and lymphatic drainage,1 time/d,the course of treatment was 1 week.The swelling degrees,pain scores among three groups at 1,3,7 d after operation were observed,the joint function scores among three groups at 1,3,7,14 d after operation were observed.Result:There were no significant differences in the swelling degrees,VAS scores and joint function score among three groups at 1 d after operation (P>0.05);there were significant differences in the swelling degrees,VAS scores at 1,3 d after operation and joint function scores at 1,3,7,14 d after operation among three groups (P<0.05).The swelling degrees and VAS scores of the observational group and lymph group at 3,7 d after operation were were lower than those of the control group,the swelling degree and VAS scores at 3,7 d after operation and VAS score at 3 d after operation in the observational group were lower than those in the lymph group,the differences were statistically significant (P<0.05).The joint function scores of the observational group and lymph group at 3,7,14 d after operation were higher than those of the control group,the joint function scores in the observational group at 3,7,14 d after operation were higher than those in the lymph group,the differences were statistically significant (P<0.05).Conclusion:Routine rehabilitation,lymphatic drainage combine with Kaixuan Prescription is effective on postoperative swelling of lower extremity joint arthroplasty surgery.
