基于“郁乃痤”理论刍议史晓伟自拟柴胡六白汤治疗痤疮

Shi Xiaowei's Self-prepared Chaihu Liubai Decoction to Treat Acne on the Basis of the Theory of“Yunaicuo”

痤疮,归属中医“粉刺”范畴,目前中医常从清热、化湿、祛瘀等方面进行诊治。“郁乃痤”为中医学对其病因病机最早的记载,并强调“阳郁”为发病的关键,但随着现代饮食、作息、情志等生活习惯的变化,加之该病发病部位特殊,大多以面部多见且病程较长,患者多生焦虑、自卑等心理。导师史晓伟主任医师长期从事内分泌疾病临床研究,擅长使用中医药治疗各类痤疮,依据中医理论结合史晓伟导师多年经验认为当下“郁乃痤”中“郁”更多指气郁,即肝气郁结,确定治法为疏肝健脾,自拟柴胡六白汤进行加减用以治疗该病。该文将基于“郁乃痤”理论探讨以疏肝健脾立法,自拟柴胡六白汤在痤疮诊疗中的运用。

Acne belongs to the category of“acne”in traditional Chinese medicine.At present,traditional Chinese medicine often diagnoses and treats from the aspects of clearing heat,removing dampness,and removing blood stasis.“Yunaicuo”is the earliest record of its etiology and pathogenesis in the motherland's medicine,and emphasizes that“yang depression”is the key to the pathogenesis.Most of them are more common in the face and have a longer course of disease,and patients are prone to anxiety and inferiority complex.Instructor Chief Physician Shi Xiaowei has been engaged in clinical research on endocrine diseases for a long time,and is good at using traditional Chinese medicine to treat various types of acne.According to the theory of traditional Chinese medicine combined with Shi Xiaowei's many years of experience,he believes that the current“Yunai Acne”in“Yu”refers to Qi stagnation,that is,liver qi stagnation.The method is to soothe the liver and invigorate the spleen,and self-made Chaihu Liubai Decoction is added and subtracted to treat the disease.Based on the theory of“Yunaicuo”,this article will discuss the application of the selfmade Chaihu Liubai Decoction in the diagnosis and treatment of acne by formulating the laws of soothing the liver and strengthening the spleen.
