杨小军治疗反流性食管炎用药规律探讨

Medication Rule of YANG Xiaojun in Treatment of Reflux Esophagitis

目的:总结杨小军教授运用中医药治疗反流性食管炎的组方用药规律。方法:收集2021年4月至2022年4月杨小军教授在门诊治疗的反流性食管炎验案,并建立反流性食管炎数据库,运用频次统计、药性、药味、归经统计、关联规则Apriori算法、系统聚类算法进行统计分析。结果:杨小军教授治疗反流性食管炎的高频药物有柴胡、陈皮、半夏、木香、枳壳、竹茹、白芍、当归、黄连、浙贝母。所涉及的中药药性以温、平、寒为主,药味以甘、辛、苦为主,所用药物主要归肺经、脾经、肝经、胃经。药物分类主要归为化痰止咳平喘药、理气药、补虚药和清热药。系统聚类得出的5个核心药组分别是:C1:黄连、紫苏梗、竹茹、浙贝母、当归、旋覆花;C2:陈皮、半夏、木香;C3:柴胡、枳壳、白芍、延胡索;C4:太子参、白术;C5:瓜蒌、海螵蛸。结论:杨小军教授在治疗反流性食管炎时注重肝、肺、脾、胃同调,益气健脾扶正气,燮理气机调肝肺,清热化痰却经邪,用药时苦辛通降。

Objective:To summarize the prescription and medication rules of Professor Yang Xiaojun using traditional Chinese medicine to treat reflux esophagitis.Methods:The cases of reflux esophagitis treated by Professor Yang Xiaojun in outpatient clinics from April 2021 to April 2022 were collected,and a reflux esophagitis database was established,using frequency statistics,drug properties,drug flavors,channel meridian statistics,and association rules Apriori Algorithms and systematic clustering algorithms for statistical analysis.Results:Professor Yang Xiaojun's high-frequency drugs for treating reflux esophagitis include Bupleurum,tangerine peel,Pinellia,Muxiang,Citrus aurantium,Zhuru,Baishao,Angelica,Coptis,and Fritillaria.The properties of the traditional Chinese medicines involved are mainly warm,flat and cold,and the flavors are mainly sweet,acrid,and bitter,and the medicines used mainly belong to the lung,spleen,liver,and stomach meridians.Drug classification is mainly classified as phlegm-relieving cough-relieving drugs,qi-regulating drugs,tonic drugs and heat-clearing drugs.The five core drug groups obtained by systematic clustering are:C1:Coptis chinensis,Perilla stem,Zhuru,Fritillaria,Angelica,Inula;C2:Chenpi,Pinellia,Muxiang;C3:Bupleurum,Citrus aurantium,Paeonia lactiflora,Corydalis;C4:Taizishen,Atractylodes;C5:Gualou,sea octopus.Conclusion:Professor Yang Xiaojun pays attention to the harmony of liver,lung,spleen and stomach in the treatment of reflux esophagitis,nourishes qi and strengthens spleen,strengthens upright qi,regulates qi and regulates liver and lung,clears heat and resolves phlegm but causes meridian pathogens.
