电针疗法对膝关节骨性关节炎患者血清TNF-α、IL-6、IL-1β水平影响的Meta分析

Meta-analysis of electroacupuncture in the treatment of knee osteoarthritis on serum levels of TNF-α,IL-6,IL-1β

目的系统评价电针治疗对膝关节骨性关节炎患者肿瘤坏死因子-α(TNF-α)、血清白介素-6(IL-6)、血清白介素-1β(IL-1β)水平的影响。方法检索CNKI、万方、CBM、维普、PubMed、Embase、Web of science及Cochrane Library数据库,纳入电针治疗膝关节骨性关节炎的临床随机对照试验,并使用RevMan 5.3软件进行Meta分析。结果纳入11个研究,共1314例膝关节骨性关节炎患者。Meta分析结果显示,电针治疗在降低患者血清TNF-α[MD=-5.54,95%CI(-7.32,-3.75),P<0.00001]、IL-6[MD=-5.03,95%CI(-7.18,-2.88),P<0.00001]、IL-1β[MD=-7.37,95%CI(-14.40,-0.34),P<0.00001]水平方面优于对照组。结论电针治疗膝关节骨性关节炎能够有效降低患者血清肿瘤坏死因子及炎症因子,且安全性较高。由于纳入文献研究结局指标数据质量不一,研究结果有待更多高质量、大样本的随机对照试验加以验证。

Objective To systematically evaluate the effect of electroacupuncture(EA)on the levels of tumor necrosis factor-α(TNF-α),serum interleukin-6(IL-6)and serum interleukin-1β(IL-1β)in patients with knee osteoarthritis.Methods The databases of CNKI,Wanfang,CBM,VIP,PubMed,Embase,Web of science and Cochrane Library were searched and included for clinical randomized controlled trials of EA in the treatment of knee osteoarthritis,and RevMan 5.3 software was used for Meta-analysis.Results Eleven studies involving 1314 patients with knee arthritis were included.The Results of Meta-analysis showed that electroacupuncture treatment was superior to the control group in terms of reduced serum TNF-α[MD=-5.54,95%CI(-7.32,-3.75),P<0.00001],IL-6[MD=-5.03,95%CI(-7.18,-2.88)],and IL-1β[MD=-7.37,95%CI(-14.40,-0.34),P<0.00001].Conclusion Electroacupuncture in the treatment of knee osteoarthritis can effectively reduce serum tumor necrosis factor and inflammatory factors,and has high safety.Due to the different quality of outcome indicators,the Results need to be verified by more high-quality and large-sample randomized controlled trials.
