ICF-CY框架下的游戏疗法在脑瘫患儿康复中的应用价值

Application value of play therapy under ICF-CY framework in rehabilitation of children with cerebral palsy

目的分析脑瘫患儿应用《国际功能、残疾和健康分类(儿童和青少年版)》(ICF-CY)框架下的游戏疗法对精细运动功能、发育水平及心理弹性的影响。方法选取2021年1月至2022年3月期间河南科技大学第一附属医院接诊的89例脑瘫患儿作为研究对象,以抽签的方法进行分组。对照组44例给予常规康复训练,观察组45例给予ICF-CY框架下游戏疗法。对比两组患儿精细运动功能、发育水平及心理弹性。结果训练后,观察组抓握(43.56±5.82)分、视觉运动(121.63±18.42)分,均高于对照组(37.63±5.74)分、(108.74±15.71)分(P<0.01);训练后,观察组精细运动(80.89±9.82)分、大运动(84.16±10.65)分、社交(82.71±11.65)分、语言(82.72±12.89)分,均高于对照组(73.67±8.24)分、(74.23±8.81)分、(73.63±10.07)分、(73.23±10.51)分(P<0.001);训练2个月末、训练3个月末,观察组心理弹性(58.97±7.05)分、(74.69±8.52)分,均高于对照组(48.85±6.89)分、(61.74±7.48)分(P<0.001)。结论ICF-CY框架下的游戏疗法能够有效提高脑瘫患儿精细运动功能,促使其正常发育,改善心理弹性。

Objective To analyze the effects of play therapy under the framework of the International Classification of Functioning,Disability and Health(Children and Youth Version)(ICF-CY)on fine motor function,developmental level and psychological resilience in children with cerebral palsy.Methods A total of 89 children with cerebral palsy admitted to the First Affiliated Hospital of Henan University of Science and Technology from January 2021 to March 2022 were selected as the research objects and divided into groups by lottery.44 cases in the control group were given routine rehabilitation training,while 45 cases in the observation group were given ICF-CY play therapy.The fine motor function,developmental level and psychological resilience were compared between the two groups.Results After training,the scores of grasping(43.56±5.82)and visuomotor(121.63±18.42)in the observation group were higher than those in the control group(37.63±5.74)and(108.74±15.71),P<0.01.After training,the scores of fine motor,gross motor,social and language in the observation group were(80.89±9.82),(84.16±10.65),(82.71±11.65)and(82.7±12.89),respectively,which were higher than those of the control group(73.67±8.24),(74.23±8.81),(73.63±10.07),(73.23±10.51),P<0.001;At the end of 2 and 3 months of training,the psychological resilience scores of the observation group were(58.97±7.05)and(74.69±8.52),which were higher than those of the control group(48.85±6.89)and(61.74±7.48),P<0.001.Conclusion Play therapy under the framework of ICF-CY can effectively improve fine motor function of children with cerebral palsy,promote their normal development,and improve psychological resilience.
