摘要 目的探讨原发免疫性血小板减少症(ITP)患者外周血淋巴细胞亚群、NKT细胞及Th1/Th2细胞因子的变化及意义。方法利用流式细胞术检测63例ITP患者(实验1组32例,PLT<20×109/L;实验2组31例,PLT≥20×109/L)及34例健康体检者(对照组)淋巴细胞亚群、NKT细胞、Th1/Th2细胞因子水平。结果ITP组CD3+、CD3+CD4+、CD3-CD16+CD56+、CD4+/CD8+、NKT水平明显低于对照组,CD3+CD8+、CD3-CD19+、Th1/Th2细胞因子(IL-2、IL-4、IL-6、IL-10、TNF-α、IFN-γ)的水平明显高于对照组(P<0.01);实验1组CD3+、CD3+CD4+、NKT水平明显低于实验2组和对照组,CD3-CD19+水平明显高于实验2组和对照组(P<0.01);ITP组NKT水平与PLT计数呈正相关(P<0.01)。结论ITP患者体内存在淋巴细胞亚群、NKT细胞及Th1/Th2细胞因子紊乱,T、B淋巴细胞亚群及NKT细胞与疾病病情密切相关。 Objective To investigate the changes and significance of lymphocyte subsets,NKT cells and Th1/Th2 cytokines in peripheral blood of patients with primary immune thrombocytopenia(ITP).Methods Peripheral blood lymphocyte subsets,NKT cells,Th1/Th2 cytokines were detected by flow cytometry in 63 ITP cases(experimental group 1,PLT<20×109/L,n=32;experimental group 2,PLT≥20×109/L,n=31)and 34 healthy controls.Results Compared with control group,the percentages of CD3+,CD3+CD4+,CD3-CD16+CD56+,CD4+/CD8+and NKT were lower,while those of CD3+CD8+,CD3-CD19+,Th1/Th2 cytokines(IL-2,IL-4,IL-6,IL-10,TNF-αand IFN-γ)were higher in ITP group(P<0.01).CD3+,CD3+CD4+and NKT cells were decreased and CD3-CD19+cells increased in experimental group 1 compared to experimental group 2 and control group(P<0.01).NKT cells were positively correlated with PLT count in ITP group(P<0.01).Conclusion Lymphocyte subsets,NKT cells and Th1/Th2 cytokines are disorganized in ITP patients,of which T/B lymphocyte subsets and NKT cells are closely related to the disease condition.
