血清同型半胱氨酸、可溶性白介素2受体水平与急性胰腺炎患者病情严重程度的关系

Relationships of serum levels of homocysteine,serum soluble interleukin 2 receptoRwith severity of illness in patients with acute pancreatitis

目的 探讨血清同型半胱氨酸(Hcy)、可溶性白介素2受体(sIL-2R)水平与急性胰腺炎(AP)患者急性生理功能和慢性健康状况评分系统Ⅱ(APACHEⅡ)评分的关系。方法 选取AP患者82例纳入AP组,另选取同期健康体检者66例纳入对照组,比较2组血清Hcy、sIL-2R水平。根据APACHEⅡ评分将82例AP患者分为轻症AP(MAP)组(APACHEⅡ评分<8分)50例和重症AP(SAP)组(APACHEⅡ评分≥8分)32例,比较2组血清Hcy、sIL-2R水平和APACHEⅡ评分。采用Pearson相关分析法分析AP患者血清Hcy、sIL-2R水平与APACHEⅡ评分的相关性。结果 AP组血清Hcy、sIL-2R水平依次为(10.75±2.05) mmol/L、(39.46±8.12) pg/mL,分别高于对照组的(6.02±0.37) mmol/L、(14.51±3.64) pg/mL,差异有统计学意义(P<0.05)。SAP组血清Hcy、sIL-2R水平和APACHEⅡ评分依次为(15.93±0.37) mmol/L、(51.32±11.09) pg/mL和(16.74±3.11)分,分别高于MAP组的(7.44±1.12) mmol/L、(31.87±6.58) pg/mL和(4.11±0.64)分,差异有统计学意义(P<0.05)。相关性分析结果显示,血清Hcy、sIL-2R水平均与APACHEⅡ评分呈正相关(P<0.05)。结论 血清Hcy、sIL-2R在AP患者中呈高表达,且与APACHEⅡ评分具有一定相关性,有望成为预测AP患者病情严重程度的辅助指标。

Objective To explore the relationships of serum homocysteine(Hcy),serum soluble interleukin 2 receptoR(SIL-2R)levels with Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)scores in acute pancreatitis(AP)patients.Methods A total of 82 AP patients were selected as AP group,and 66 healthy subjects were selected as control group.Serum levels of Hcy and sIL-2Rwere compared between the two groups.According to APACHEⅡscore,they were divided into mild AP group(MAP group,n=50,APACHEⅡscore<8),severe AP group(SAP group,n=32,APACHEⅡscore≥8).The correlations of Hcy and SIL-2Rwith APACHEⅡscore were analyzed by Pearson method.Results The levels of serum Hcy and SIL-2Rin the AP group were(10.75±2.05)mmol/L,(39.46±8.12)pg/mL,respectively,which were significantly higheRthan(6.02±0.37)mmol/L and(14.51±3.64)pg/mL in the control group(P<0.05).Serum Hcy and SIL-2Rlevels and APACHEⅡscore in the SAP group were(15.93±0.37)mmol/L(51.32±11.09)pg/mL and(16.74±3.11)points,which were higheRthan(7.44±1.12)mmol/L,(31.87±6.58)pg/mL and(4.11±0.64)points in the MAP group(P<0.05).Correlation analysis showed that serum Hcy,sIL-2Rwere positively correlated with APACHEⅡscore(P<0.05).Conclusion Serum Hcy and sIL-2Rare highly expressed in AP patients,and have a certain correlation with the APACHEⅡscore,which are expected to become auxiliary indicators foRpredicting the severity of AP patients.
