血必净联合激素在脓毒血症急诊处理中的疗效研究

Study on Curative Effect of Xuebijing Combined with Hormone in Emergency Treatment of Sepsis

目的研究血必净联合激素治疗脓毒血症的急诊处理疗效。方法选取2018年3月-2021年6月在我院诊治的46例脓毒血症患者为研究对象,采用随机数字表法分为对照组和观察组,各23例。对照组给予常规治疗和氢化可的松治疗,观察组在对照组基础上给予血必净治疗,比较两组治疗总有效率、炎症因子水平、并发症发生率、急性生理学及慢性健康状况(APACHEⅡ)评分及30 d存活率。结果观察组治疗总有效率为91.30%,高于对照组的82.61%(P<0.05);两组治疗后C-反应蛋白(CRP)、降钙素原(PCT)、白细胞计数(WBC)均低于治疗前,且观察组低于对照组(P<0.05);观察组并发症发生率为8.70%,与对照组的13.04%比较,差异无统计学意义(P>0.05);两组治疗后APACHEⅡ评分均低于治疗前,且观察组低于对照组(P<0.05);观察组30 d存活率为95.65%,与对照组的86.95%比较,差异无统计学意义(P>0.05)。结论血必净联合激素在脓毒血症急诊处理中具有确切的效果,可提高治疗有效率,减轻炎症因子水平,减少并发症发生,存活率高,是一种安全、有效的处理方法,值得临床应用。

Objective To study the emergency treatment effect of Xuebijing combined with hormone in the treatment of sepsis.Methods A total of 46 patients with sepsis diagnosed and treated in our hospital from March 2018 to June 2021 were selected as the research objects.They were divided into control group and observation group by random number table method,with 23 cases in each group.The control group was treated with routine treatment and hydrocortisone,and the observation group was treated with Xuebijing on the basis of the control group.The total effective rate,inflammatory factor level,complication rate,acute physiology and chronic health status(APACHEⅡ)score and 30-day survival rate were compared between the two groups.Results The total effective rate of treatment in the observation group was 91.30%,which was higher than 82.61%in the control group(P<0.05).After treatment,C-reactive protein(CRP),procalcitonin(PCT)and white blood cell count(WBC)in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).The incidence of complications in the observation group was 8.70%,which was compared with 13.04%in the control group,the difference was not statistically significant(P>0.05).The APACHE II scores of the two groups after treatment were lower than those before treatment,and the observation group was lower than the control group(P<0.05).The 30-day survival rate of the observation group was 95.65%,which was compared with 86.95%of the control group,the difference was not statistically significant(P>0.05).Conclusion Xuebijing combined with hormone has definite effect in emergency treatment of sepsis,which can improve the effective rate of treatment,reduce the level of inflammatory factors,reduce the incidence of complications,and high survival rate.It is a safe and effective treatment method,which is worthy of clinical application.
