摘要
目的:观察64排螺旋CT低剂量扫描检查在肺癌高危人群早期肺癌诊断中的效果。方法:选取2020年1月至2022年3月该院收治的368例肺癌高危患者进行前瞻性研究,按随机数字表法将其分为观察组(n=183)和对照组(n=185)。观察组采用肺部CT低剂量扫描检查,对照组采用肺部CT常规剂量扫描检查,比较两组早期肺癌检出率、肺部结节检出情况、CT放射检查指标[扫描总电流时间积(mAs)、权重CT剂量指数(CTDIW)、有效扫描mAs、剂量长度乘积(DLP)、最小红斑量(MERD)]水平、图像质量。结果:两组早期肺癌检出率比较,差异无统计学意义(P>0.05);两组钙化、空洞、分叶、毛刺、结节检出率及结节直径比较,差异均无统计学意义(P>0.05);观察组CTDIW、扫描总mAs、DLP、MERD水平均低于对照组,有效扫描mAs水平高于对照组,差异均有统计学意义(P<0.05);两组图像质量优良率比较,差异无统计学意义(P>0.05)。结论:64排螺旋CT低剂量扫描检查应用于肺癌高危人群早期肺癌诊断中可降低辐射水平,提高有效扫描mAs水平,且不会降低检出率。
Objective: To observe effects of low-dose 64-slice spiral CT scan in diagnosis of early lung cancer in high-risk group of lung cancer. Methods: 368 high-risk patients with lung cancer who were admitted to this hospital from January 2020 to March 2022 were selected for the prospective study, and were divided into observation group(n=183) and control group(n=185) according to the random number table method. The observation group was examined by low-dose lung CT scan, while the control group was examined by conventional dose lung CT scan. The detection rate of early lung cancer, the detection of pulmonary nodules, the CT radiological examination indicator levels [total scan current time product(mAs),weighted CT dose index(CTDIW), effective scan mAs, dose length product(DLP), minimum erythema dose(MERD)], the image quality good and excellent rate were compared between the two groups. Results: There was no significant difference in the detection rate of early lung cancer between the two groups(P>0.05). There were no significant difference in the detection rates of calcification, cavity, lobulation, burr, nodule and the nodule diameter between the two groups(P>0.05). The levels of CTDIW, total scanning mAs, DLP, and MERD in the observation group were lower than those in the control group;the effective scanning mAs were higher than those in the control group;and the differences were statistically significant(P<0.05).There was no significant difference in the scanning image excellent and good rate between the two groups(P>0.05). Conclusions: Low-dose 64-slice spiral CT scanning in the diagnosis of early lung cancer in high-risk group of lung cancer can reduce the radiation levels and the low-dose levels.Moreover, it is superior to conventional dose scan.
