新型诊疗模式下神外临床教学的应对和思考

摘要 神经外科接诊的疾病谱较广,患者病情复杂且进展较快,发病部位凶险,需要高度复杂的外科治疗手段,要求医师基于大量的临床手术实践以积累经验来满足患者对优质神经外科手术的需求。在传染病的短期暴发期间,为满足传染病防控的要求或降低医院成为交叉感染平台的风险可以采取远程学习以临时应对,但当进入长期的常态化防控后,神外科诊疗模式在防控措施的影响下发生了转变,增加了入院传染病筛查、隔离-过渡病房观察、线上会议或问诊等诊疗流程。如何在新型诊疗模式下进行神外临床教学以满足工作要求和神经外科“学生-医师”转变所需的技能和经验成为我们必须解决的问题。文章拟从剖析新发传染病和新型诊疗模式的基本特征出发,分析两者之间的关联及其对神外临床教学的影响,从课程安排、科技辅助和线上教学数据保密等角度探讨新形势下神经外科教学的有效应对方式和具体途径。 Neurosurgery has a broad spectrum of diseases.The patients with complex and fast-developing conditions,and the site of the disease is dangerous,so that requires highly sophisticated surgical treatment.The training of its doctors is based on a large number of clinical practice to accumulate experience to meet the needs of patients for high-quality neurosurgery.During the short-term outbreak of infectious diseases,long-distance learning can be used temporarily to meet the requirements of infection disease prevention and control or to reduce the risk of hospitals becoming a platform for cross-infection.But after entering into long-term normal prevention and control,the mode of neurosurgical diagnosis and treatment has changed under the influence of epidemic prevention and control measures,with the addition of diagnostic and treatment procedures such as inpatient infectious disease screening,isolation-transition ward observation,online meetings or consultations.How to carry out the clinical teaching of neurosurgery in the new mode of diagnosis and treatment to meet the requirements of prevention and the skills and experience needed by the"student-doctor"transformation of neurosurgery has become a problem that we must solve.The purpose of this article is to analyze the relationship between the emerging infectious diseases and the new diagnostic and therapeutic model,and their impact on clinical teaching,to explore the effective ways and means of neurosurgery teaching under the new situation from the perspectives of curriculum arrangement,science and technology assistance and online teaching data confidentiality.
