腹腔镜肾窦内肾盂切开取石术治疗肾盂单发结石的疗效观察

Observation of the Curative Effect of Laparoscopic Intrarenal Pyelectomy for Single Renal Pelvic Stones

目的:分析肾盂单发结石疾病特点,评价腹腔镜肾窦内肾盂切开取石术治疗优势。方法:选择2019年5月~2020年5月本院泌尿外科的84例肾盂单发结石疾病患者为观察对象,采取1:1比例法分为观察组与对照组。对照组患者采取经皮肾镜碎石取石术治疗,观察组患者采取腹腔镜肾窦内肾盂切开取石术治疗。比较2组肾盂单发结石疾病患者的手术相关指标、取石成功率、并发症发生情况以及血清炎症指标水平。结果:组间术中出血量以及住院时间比较,观察组患者指标均值更低(P<0.05);组间术后1个月取石成功率比较,观察组取石成功率97.61%明显高于对照组(P<0.05);组间术后尿瘘、发热、尿路感染并发症发生情况比较,观察组患者并发症发生率2.39%低于对照组(P<0.05);组间白细胞介素-6(Interleukin-6)、超敏C反应蛋白(hypersensitive C-Reactive Protein,hs-CRP)、肿瘤坏死因子(Umor Necrosis Factor-α,TNF-α)比较,术后观察组均低于对照组(P<0.05)。结论:肾盂单发结石具有多发趋势,与经皮肾镜碎石取石术比较,腹腔镜肾窦内肾盂切开取石术治疗更具安全性、有效性价值。

Objective:To analyze the characteristics of single renal pelvic stone disease,and to evaluate the therapeutic advantages of laparoscopic intrarenal pyelectomy.Methods:Select 84 patients with single renal pelvic stone disease from May 2019 to May 2020 in the Department of Urology in our hospital,and adopt the 1:1 ratio method.Patients in the control group were treated with percutaneous nephrolithotomy,and patients in the observation group were treated with laparoscopic intrarenal pyelectomy.Comparison of the two groups of patients with renal pelvic single stone disease related to surgery,stone retrieval success rate,complications and serum inflammation index levels.Results:Comparing intraoperative blood loss and hospital stay between the groups,the observation group had fewer average indicators(P<0.05);The comparison of the success rate of stone retrieval 1 month after operation between the groups,the success rate of 97.61%in the observation group was significantly higher than that of the control group(P<0.05);Comparing the postoperative complications of urinary fistula,fever,and urinary tract infection between the groups,the complication rate of the observation group was 2.39%lower than that of the control group(P<0.05);The inter-group interleukin-6(Interleukin-6),hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)comparison,the observation group was lower than the control group after operation(P<0.05).Conclusion:Single renal pelvis stones have a tendency to occur frequently.Compared with percutaneous nephrolithotomy,laparoscopic intrarenal pelvic lithotripsy is safer and more effective.
