彩色多普勒超声对胆囊息肉样病变的鉴别诊断价值

Value of Color Doppler Ultrasonography in Differential Diagnosis of Gallbladder Polypoid Lesions

目的:探究胆囊息肉样病变患者行彩色多普勒超声检查对疾病的鉴别诊断价值。方法:选择2019年8月~2021年8月时间段内,于本院接受彩色多普勒检查的胆囊息肉样病变患者112例为研究对象,其中胆囊息肉75例、胆囊腺瘤13例,胆囊腺肌增生症8例,小结节性胆囊癌16例。所有患者均给予彩色多普勒超声诊断,使用回顾性分析方法分析彩色多普勒超声诊断结果,评判其诊断价值。结果:小结节性胆囊癌患者即恶性病变患者,彩色多普勒血流显像结果可见丰富血流,与胆囊息肉、胆囊腺瘤等良性病变相比,彩色多普勒血流图的血流显示阳性率、收缩期最大血流速度(peak systolic velocity,PSV)和阻力指数(resistance index,RI)对比结果,差异有统计学意义(P<0.05)。结论:胆囊息肉样病变诊断中,如二维超声信息完整、清晰,包括病变部位、大小、形态、数目等,综合彩色多普勒超声检查结果,包括血流性质、Vmax、RI等,可进一步提高诊断准确率,具有较高的临床应用价值。

Objective:To explore the differential diagnosis value of color Doppler ultrasonography in patients with gallbladder polypoid lesions.Methods:A total of 112 patients with gallbladder polypoid lesions who received color Doppler examination in our hospital from August 2019.08 to August 2021.08 were selected as the study subjects,including 75 cases of gallbladder polyp,13 cases of gallbladder adenoma,8 cases of gallbladder adenomyosis and 16 cases of small nodular gallbladder carcinoma.All patients were diagnosed by color Doppler ultrasound,and the diagnostic value of color Doppler ultrasound was evaluated by retrospective analysis.Results:The patients with small nodular gallbladder or malignant lesions,color Doppler flow imaging result shows rich blood flow,and gallbladder polyp and gallbladder adenoma compared to the benign lesions such as color Doppler blood flow diagram shows positive rate,blood flow to the Vmax(systolic maximum blood flow velocity)and RI(resistance index)comparing the results,the difference was statistically significant(P<0.05).Conclusion:In the diagnosis of gallbladder polypoid lesions,if two-dimensional ultrasound information is complete and clear,including lesion location,size,shape,number,etc.,and the results of color Doppler ultrasound examination,including blood flow properties,Vmax,RI,etc.,can further improve the diagnostic accuracy and have high clinical application value.
