摘要
目的:探究无创呼吸机辅助通气治疗重症肺炎致呼吸衰竭的临床应用效果。方法:选择2019年9月~2021年9月本院收治的重症肺炎致呼吸衰竭患者80例为观察对象,依据不同治疗方式划分成观察组(无创呼吸机辅助通气治疗方式)及对照组(常规治疗方式),各40例。对比患者的呼吸功能指标、临床疗效、不良反应发生率、呼吸频率和心率、肺功能指标等情况。结果:与对照组的各项呼吸功能指标对比,观察组的a/APO_(2)、pH、PaO_(2)较高,但是OI、PaCO_(2)较低,差异有意义(P<0.05);与对照组的治疗总有效率70.00%(28例)比较,观察组的95.00%(38例)明显更高,差异有意义(P<0.05);与对照组发生的不良反应(20.00%)比较,观察组的误吸、口鼻干燥、排痰障碍等发生率(2.50%)显著较低,差异有意义(P<0.05);与对照组的FEV1/FVC、FEV1%pred指标相比,观察组均明显较高,差异有意义(P<0.05);与对照组的呼吸频率和心率指标相比,观察组均明显较低,差异有意义(P<0.05)。结论:针对重症肺炎致呼吸衰竭患者,通过采用无创呼吸机辅助通气治疗的方式,能够改善患者的肺部功能指标,改善疾病预后,降低不良反应发生率,改善呼吸功能指标和呼吸频率、心率。
Objective:To explore the clinical effect of noninvasive ventilator-assisted ventilation in the treatment of respiratory failure caused by severe pneumonia.Methods:80 patients with respiratory failure caused by severe pneumonia treated in our hospital from September 2019 to September 2021 were selected as the observation objects.They were divided into observation group(noninvasive ventilator-assisted ventilation treatment)and control group(routine treatment),with 40 cases in each group.The respiratory function indexes,clinical efficacy,incidence of adverse reactions,respiratory rate,heart rate and pulmonary function indexes were compared.Results:Compared with the control group,the observation group had higher a/APO_(2),pH and PaO_(2),but lower OI and PaCO_(2)(P<0.05);Compared with 28 cases(70.00%)in the control group,38 cases(95.00%)in the observation group were significantly higher(P<0.05);Compared with 8 cases(20.00%)in the control group,1 case(2.50%)in the observation group had a significantly lower incidence of aspiration,dry mouth and nose and sputum excretion disorder(P<0.05);Compared with the FEV1/FVC and FEV1%pred indexes of the control group,the indexes of the observation group were significantly higher(P<0.05);Compared with the control group,the respiratory rate and heart rate in the observation group were significantly lower(P<0.05).Conclusion:For patients with respiratory failure caused by severe pneumonia,noninvasive ventilator assisted ventilation can improve the pulmonary function index,improve the disease prognosis,reduce the incidence of adverse reactions,improve the respiratory function index,respiratory rate and heart rate.
