肠外营养联合肠内营养对肺癌术后化疗患者营养状况及免疫功能的影响 被引量:3

Effects of parenteral nutrition combined with enteral nutrition on nutritional status and immunologic function in patients with postoperative chemotherapy for lung cancer

摘要 目的探讨肠外营养联合肠内营养对肺癌术后化疗患者营养状况及免疫功能的影响。方法选取我院收治的110例肺癌术后化疗患者作为研究对象,采用随机数字表法分为对照组(55例)和观察组(55例)。对照组给予肠内营养治疗,观察组在对照组基础上给予肠外营养治疗,2组患者均治疗14 d。比较2组患者治疗前、治疗14 d后的营养指标、免疫球蛋白水平、T淋巴细胞水平,并记录2组患者手术切口愈合时间及并发症发生率。结果与治疗前相比,治疗14 d后2组患者血清总蛋白(TP)水平及观察组血清白蛋白(ALB)、前白蛋白(PA)、免疫球蛋白G(IgG)、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、CD56^(+)水平均升高(P<0.05),且观察组均高于对照组(P<0.05)。治疗期间,观察组手术切口愈合时间短于对照组(P<0.05);2组患者切口感染、切口出血发生率比较,差异无统计学意义(P>0.05)。结论肠外营养联合肠内营养治疗能够有效改善肺癌术后化疗患者的营养状况,进一步提高患者免疫功能,提升患者术后恢复效果,安全性良好。 Objective To investigate the effects of parenteral nutrition combined with enteral nutrition on nutritional status and immunologic function in patients with postoperative chemotherapy for lung cancer.Methods A total of 110 patients with postoperative chemotherapy for lung cancer in our hospital were selected as the research objects and they were divided into the control group(55 cases)and the observation group(55 cases)by random number table method.The control group was treated with enteral nutrition,and the observation group was treated with parenteral nutrition on the basis of the control group,patients in both groups were treated for 14 days.The nutritional indexes,immunoglobulin level and T lymphocyte level before treatment and 14 days after treatment of the two groups were compared,and the time of surgical incision healing and the incidence of complications of the two groups were recorded.Results Compared with those before treatment,the levels of serum total protein(TP)of the two groups and the levels of serum albumin(ALB),prealbumin(PA),immunoglobulin G(IgG),CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and CD56^(+)in the observation group 14 days after treatment were increased,and the levels in the o bservation group were higher than those in the control group(P<0.05).During the treatment period,the time of surgical incision healing in the observation group was shorter than that in the control group(P<0.05).There was no statistically significant difference in the incidence of incision infection and incision bleeding between the two groups(P>0.05).Conclusion Parenteral nutrition combined with enteral nutrition can effectively improve the nutritional status of patients with postoperative chemotherapy for lung cancer,further improve the immunologic function of patients,and then enhance the postoperative recovery effect of patients,with good safety.
