肝硬化合并非静脉曲张性上消化道出血的临床分析

Clinical analysis of cirrhosis with non-variceal upper gastrointestinal bleeding

目的分析肝硬化合并非静脉曲张性上消化道出血的临床特征。方法88例肝硬化患者,将其中44例肝硬化合并非静脉曲张性上消化道出血患者作为观察组,另外44例肝硬化合并静脉曲张性上消化道出血患者作为参照组。两组患者均进行常规实验室检查,比较两组患者的实验室检查结果、合并症发生情况。结果观察组尿素氮(BUN)(9.08±1.17)mmol/L高于参照组的(6.37±0.99)mmol/L,差异有统计学意义(P<0.05);两组血小板计数(PLT)、白蛋白(ALB)、血红蛋白(HGB)、肌酐(Cr)水平比较,差异无统计学意义(P>0.05)。观察组冠心病、高血压、十二指肠溃疡、胃溃疡占比分别为22.73%、18.18%、59.09%、27.27%,均高于参照组的6.82%、4.55%、36.36%、9.09%,差异有统计学意义(P<0.05)。结论肝硬化合并非静脉曲张性上消化道出血患者BUN水平会相对较高,且容易存在各种严重合并症,临床在为该类患者诊疗时应该予以高度重视。

Objective To analyze the clinical features of cirrhosis with non-variceal upper gastrointestinal bleeding.Methods Of the 88 patients with cirrhosis,44 patients with cirrhosis and non-variceal upper gastrointestinal bleeding were selected as the observation group,and the other 44 patients with cirrhosis and variceal upper gastrointestinal bleeding were selected as the reference group.Routine laboratory tests were performed in both group,and the laboratory test results and the occurrence of comorbidities were compared between the two groups.Results The blood urea nitrogen(BUN)(9.08±1.17)mmol/L in the observation group was higher than(6.37±0.99)mmol/L in the reference group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in platelet count(PLT),albumin(ALB),hemoglobin(HGB),and creatinine(Cr)levels between the two groups(P>0.05).The percentages of coronary heart disease,hypertension,duodenal ulcer,and gastric ulcer in the observation group were 22.73%,18.18%,59.09%,and 27.27%,which were higher than 6.82%,4.55%,36.36%,and 9.09%in the reference group,and the differences were all statistically significant(P<0.05).Conclusion Patients with cirrhosis and non-variceal upper gastrointestinal bleeding usually have relatively high BUN levels and are prone to serious comorbidities.Therefore,great attention should be paid to the clinical diagnosis and treatment of such patients.
