中枢神经系统散发性血管母细胞瘤MRI征象与病理对照

MRI Features of Sporadic Hemangioblastomas of Central Nervous System Compared with Pathological Findings

摘要 目的探讨中枢神经系统散发性血管母细胞瘤(sHBM)MRI征象,并与病理对照分析,提高诊断及鉴别诊断能力。方法收集蚌埠医学院第一附属医院2011年1月至2019年7月16例手术病理证实的sHBM进行回顾性研究。结果本组sHBM均为单发,周围可见范围不等水肿带环绕。囊结节型9例,实性肿块型7例。壁结节和实性肿块T1WI等或稍低信号,部分内见斑片状出血;T2WI等、稍高或高信号,DWI等或低信号。增强扫描,壁结节和实性肿块显著强化,囊腔、囊壁未见明显强化。镜下,壁结节和实性肿块由间质细胞及毛细血管构成,免疫组化CD34、CD31、Vimentin、NSE、S100、CD56、GFAP和Ki-67阳性表达。结论MRI对中枢神经系统sHBM具有重要诊断价值。 Objective To explore the MRI features of sporadic hemangioblastoma(sHBM)of central nervous system and to compare them with pathology so as to improve the ability of diagnosis and differential diagnosis.Methods 16 cases of sHBM confirmed by operation and pathology in the First Affiliated Hospital of Bengbu Medical College from January 2011 to July 2019 were collected for retrospective study.Results 16 cases of sHBMs were all single,there being minimal to strong edema surrounding the HBMs.There were 9 cases of cyst-nodule type sHBMs and 7 cases of solid-mass type sHBMs respectively.On T_(1)WI,the mural nodules and solid masses mainly demonstrated iso-or slightly hypointense signal,and some of them showed patchy bleeding signal,while T_(2)WI demonstrated iso-,slightly hyper-or hyperintense signal,DWI showed iso-or hypointense signal.Enhanced scan,the mural nodules and solid masses were strongly enhanced,with no obvious enhancement of the cyst cavity and wall.Microscopically,the mural nodules and solid masses were composed of characteristic stromal cells and abundant capillaries.CD34,CD31,Vimentin,NSE,S100,CD56,GFAP and Ki-67 in most of the specimen were positive.Conclusion MRI has important diagnostic value for central nervous system sHBM.
