多囊卵巢综合征青春期月经特点及其诊断意义

Adolescent menstrual characteristics and diagnostic significance of polycystic ovary syndrome

目的探讨青春期月经特点对日后多囊卵巢综合征(PCOS)、PCOS合并代谢综合征(MS)的预测作用。方法选择2017年7月至2019年9月在大连大学附属中山医院就诊的85例PCOS患者作为研究组,根据是否合并MS分为代谢异常组(26例)、代谢正常组(59例)。同期健康女性80名作为对照组,分析各组月经规律及对日后发生PCOS、PCOS合并MS的预测意义。结果代谢异常组与代谢正常组的月经异常率比较,差异无统计学意义(P>0.05);研究组与对照组在初潮后3、4年内及16~19岁时月经异常率比较,差异均有统计学意义(P<0.05)。结论青春期女性从初潮后3年开始月经异常对于日后发生PCOS有预测作用,并随年龄增加预测作用逐渐增大。PCOS女性青春期月经特点对日后MS的发生没有预测作用。

Objective To explore the predictive effect of adolescent menstrual characteristics on polycystic ovary syndrome(PCOS)and PCOS combined with metabolic syndrome(MS)in the future.Methods A total of 85 patients with PCOS who were treated in Affiliated Zhongshan Hospital of Dalian University from July 2017 to September 2019 were selected as the research group.According to whether they were combined with MS,they were divided into 26 cases of abnormal metabolism group and 59 cases of normal metabolism group.During the same period,80 healthy women were selected as the control group.The menstrual regularity of each group and its predictive significance for the occurrence of PCOS and PCOS combined with MS in the future were analyzed.Results There was no significant difference in menstrual abnormalities rate between the abnormal metabolism group and the normal metabolism group(P>0.05).There were statistically significant differences in menstrual abnormalities between the study group and the control group within 3 and 4 years after menarche and at 16-19 years old(P<0.05).Conclusion The abnormal menstruation in adolescent women from 3 years after menarche has a predictive effect on the occurrence of PCOS in the future,and the predictive effect gradually increases with age.The menstrual characteristics of PCOS women during puberty have no predictive effect on the occurrence of MS in the future.
