麻醉护士运用精益管理理念提升心脏体外循环手术护理质量的效果评价

Effect evaluation of anesthesia nurses applying the concept of lean management to optimize the nursing quality of cardiopulmonary bypass surgery

目的探讨麻醉护士运用精益管理理念在心脏体外循环手术中的应用效果。方法选取2020年1月—2022年1月于医院行心脏体外循环手术的患者128例,按照组间基本特征均衡可比的原则分为对照组和观察组,各64例。对照组采取传统麻醉护理措施,观察组在对照组基础上采取精益管理理念指导麻醉护理。观察至患者出院,比较两组术后康复情况、并发症发生率和满意度。结果观察组首次排气时间(24.35±3.42 h)、首次排便时间(47.86±5.19 h)、术后拔管时间(2.79±0.53 d)和术后住院时间(18.57±2.60 d)均短于对照组,组间比较差异具有统计学意义(P<0.05);观察组术后并发症发生率为3.13%,低于对照组的15.63%,组间比较差异具有统计学意义(P<0.05);观察组护理满意度中护理方式(86.97±3.35分)、操作技术(85.99±3.30分)、服务态度(87.78±3.38分)和护理安全(86.95±3.42分)均高于对照组,组间比较差异具有统计学意义(P<0.05)。结论麻醉护士将精益管理理念运用于心脏体外循环手术中,能够降低并发症发生率,加快患者术后康复进程,从而提升患者满意度。

Objective To explore the application effect of anesthesia nurses’ lean management concept in cardiopulmonary bypass surgery. Methods A total of 128 patients who underwent cardiopulmonary bypass in hospital from January 2020 to January 2022 were selected and divided into control group and observation group according to the principle of balanced and comparable basic characteristics between groups, with 64 patients in each group. The control group took traditional anesthetic nursing measures, while the observation group took lean management concept to guide anesthetic nursing on the basis of the control group. After the patients were discharged from hospital, the postoperative rehabilitation, complication rate and satisfaction of the two groups were compared. Results The time offirst exhaust(24.35±3.42 hours), time offirst bowel movement(47.86±5.19 hours), extubation time(2.79±0.53 days) and hospital stay(18.57±2.60 days) in the observation group were shorter than those in the control group, and the differences between the groups were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 3.13%, lower than 15.63% in the control group, and the difference between the groups was statistically significant(P<0.05). The nursing mode(86.97±3.35 points),operation technique(85.99±3.30 points), service attitude(87.78±3.38 points) and nursing safety(86.95±3.42 points) in the observation group were higher than those in the control group, and the differences between the groups were statistically significant(P<0.05). Conclusion The application of the concept of lean management in cardiopulmonary bypass surgery by anesthesia nurses can reduce the incidence of complications, speed up the process of postoperative rehabilitation, and improve patient satisfaction.
