摘要
代谢相关脂肪性肝病(metabolic associated fatty liver disease,MAFLD)是常见的肝脏疾病,近年来先天免疫的活化在MAFLD进展中的作用被大量研究证实,Toll样受体(Toll like receptor,TLR)途径是其中关键的一环。TLR在各种肝脏疾病中均有异常表达,其存在可加重肝脏发生炎症反应,其发挥作用主要是通过调节TNF-α和IL-1β表达实现的。肠道菌群的构成与功能的改变通过TLR特异性识别脂多糖,并激活炎症相关基因的活性,促使肝脏慢性炎症的产生,最终导致MAFLD的发生发展。益生菌可以通过调节肠道菌群降低TLR的表达,进而改善机体炎症刺激,改善MAFLD。
Metabolic associated fatty liver disease(MAFLD)has become the most common liver disease.In recent years,the role of immune activation in the progression of MAFLD has been confirmed by a large number of studies,in which Toll like receptor(TLR)pathway is a key link.TLR is abnormally expressed in various liver disease,its existence can aggravate the inflammatory reaction in the liver,its role is mainly realized by regulating the expressions of TNF-αand IL-1β.Changes in the composition and function of gut microbiota specifically recognize lipopolysaccharide through-TLR,activate the activity of inflammation related genes,promote the generation of chronic liver inflammation,and finally lead to the occurrence and development of MAFLD.Probiotics can reduce the expression of TLR by regulating gut microbiota,thereby improving the inflammatory stimulation of the body and MAFLD.
