早期拔除胃管在老年食管癌加速康复外科治疗中的效果观察

Clinical observation on the effect of early removal of gastric tube in enhanced recovery after surgery for elderly esophageal cancer

目的探讨早期拔除胃管在老年食管癌加速康复外科(enhanced recovery after surgery,ERAS)治疗中的有效性及安全性。方法选取郑州大学第二附属医院2018年1月至2021年12月行食管癌ERAS治疗的老年患者54例,将其分为早期拔除胃管组(n=25)和传统方法组(n=29)。比较两组患者术后首次排气时间、术后住院时间及术后并发症发生情况。结果早期拔除胃管组较传统方法组患者手术后首次排气时间提前,住院时间缩短,两组比较,差异有统计学意义(P<0.05)。早期拔除胃管组较传统方法组患者手术后并发症发生率低;两组术后肺炎、心律失常发生率比较,差异有统计学意义(P<0.05);两组术后血栓、切口感染发生率比较,差异无统计学意义(P>0.05)。结论早期拔除胃管在老年食管癌ERAS治疗中可以促进术后更快恢复,缩短住院时间。

Objective To investigate the effectiveness and safety of early removal of gastric tube in enhanced recovery after surgery(ERAS)for elderly esophageal cancer.Methods A total of 54 cases of esophageal cancer operation in the Second Affiliated Hospital of Zhengzhou University from Jan.2018 to Dec.2021 were selected and divided into the early removal of gastric tube group(n=25)and the traditional group(n=29).The postoperative first time exhaust and defecation time,hospitalization stay length and postoperative complications were compared between the two groups.Results Compared with the patients in the traditional group,the expulsion time for the first time after the operation was earlier in early removal of gastric tube group and the length of hospital stay was reduced.The difference between the two groups was statistically significant(P<0.05).The incidence of postoperative complications in the early removal of gastric tube group was lower than that in the traditional group;the incidence of postoperative pneumonia and arrhythmia between the two groups was statistically significant(P<0.05);the incidence of postoperative thrombus and incision infection was not statistically significant(P>0.05).Conclusion Early removal of the gastric tube in ERAS for elderly esophageal cancer can promote faster recovery after surgery and shorten the length of hospital stay.
