慢性肾衰腹膜透析患者衰弱发生的影响因素

Influencing Factors of Weakness in Patients with Chronic Renal Failure After Peritoneal Dialysis

目的分析慢性肾衰腹膜透析患者衰弱发生的影响因素。方法选取2019年1月至2021年12月在河南科技大学第一附属医院接受腹膜透析治疗的420例患者,统计患者腹膜透析后衰弱发生情况,询问并记录患者基线资料,设计基线资料填写表,分析慢性肾衰患者腹膜透析后发生衰弱的影响因素。结果腹膜透析治疗后,420例慢性肾衰患者中发生衰弱53例,占比为12.62%;与未发生组对比,发生组透析不充分和透析低血压比率高,血清C反应蛋白(CRP)水平高,血清白蛋白水平低(P<0.05);两组在透析时间、合并糖尿病、合并高血压等方面比较,差异无统计学意义(P>0.05);经logistic回归分析,结果显示,透析不充分、透析低血压、血清CRP水平高是慢性肾衰患者发生衰弱的危险因素(OR>1,P<0.05),血清白蛋白水平高是其保护因素(OR<1,P<0.05)。结论慢性肾衰患者腹膜透析后发生衰弱的影响因素可能为透析不充分、透析低血压、血清CRP水平高、血清白蛋白水平高。

Objective To analyze the influencing factors of weakness in patients with chronic renal failure after peritoneal dialysis.Methods A total of 420 patients who received peritoneal dialysis in the Affiliated Hospital of Henan University of Science and Technology from January 2019 to December 2021 were selected.The occurrence of weakness after peritoneal dialysis was counted.Asked and recorded the baseline data of patients,then designed the baseline data filling form.The influencing factors of weakness after peritoneal dialysis in patients with chronic renal failure were analyzed.Results After peritoneal dialysis,53 of 420 patients with chronic renal failure developed weakness,accounting for 12.62%.Compared with the non occurrence group,the proportions of dialysis insufficiency and dialysis hypotension in the occurrence group were higher,the level of serum C-reactive protein(CRP)was higher,and the level of serum albumin was lower(P<0.05).There was no significant difference between the two groups in dialysis time,diabetes mellitus and hypertension(P>0.05).The results of logistic regression analysis showed that,insufficient dialysis,low blood pressure and high serum CRP level were the risk factors of weakness in patients with chronic renal failure(OR>1,P<0.05),and high serum albumin level was the protective factor(OR<1,P<0.05).Conclusion The influencing factors of weakness after peritoneal dialysis in patients with chronic renal failure may be insufficient dialysis,dialysis hypotension,high serum CRP level and high serum albumin level.
