系统性红斑狼疮并发骨坏死与C反应蛋白、免疫球蛋白G和血细胞比容的相关性

Correlation Between Systemic Lupus Erythematosus Complicated with Osteonecrosis and C-Reactive Protein,Immunoglobulin G and Hematocrit

目的探讨系统性红斑狼疮(SLE)并发骨坏死的特点及与C反应蛋白(CRP)、免疫球蛋白G(IgG)和血细胞比容(HCT)的相关性。方法选取郑州中医骨伤病医院2019年5月至2021年6月收治的91例SLE患者为研究对象,根据有无发生骨坏死分组,26例发生骨坏死患者设为观察组,65例未发生骨坏死患者设为对照组。比较两组临床表现、血清CRP、IgG、HCT水平,分析血清CRP、IgG、HCT与SLE并发骨坏死的关系。结果观察组患者皮肤血管炎、肌痛肌无力、骨质疏松占比高于对照组(P<0.05)。观察组患者血清CRP、IgG水平高于对照组(P<0.05),观察组患者HCT比率低于对照组(P<0.05)。血清CRP、IgG与SLE并发骨坏死呈正相关(r=0.752、0.787,P<0.001);血清HCT与SLE并发骨坏死呈负相关(r=-0.812,P<0.001)。结论SLE患者发生骨坏死与血清IgG、CRP、HCT水平有关,血清IgG、CRP、HCT在鉴别SLE患者并发骨坏死及病情程度中具有重要意义,值得推广。

Objective To investigate the characteristics of osteonecrosis of systemic lupus erythematosus(SLE)and its correlation with C-reactive protein(CRP),immunoglobulin G(IgG)and hematocrit(HCT).Methods Ninety-one patients with SLE admitted to Zhengzhou Traditional Chinese Medicine Orthopaedic Hospital from May 2019 to June 2021 were selected as the research objects,and were divided into groups according to the presence or absence of osteonecrosis.A total of 26 patients with osteonecrosis were set as the observation group,and 65 patients without osteonecrosis were set as the control group.The clinical manifestations,serum CRP,IgG,HCT levels were compared between the two groups,and the relationship between serum CRP,IgG,HCT and SLE complicated with osteonecrosis was analyzed.Results The proportions of cutaneous vasculitis,myalgia and muscle weakness of osteoporosis in observation group were higher than those in control group(P<0.05).The serum CRP and IgG levels in observation group were higher than those in control group(P<0.05).The proportion of HCT in observation group was lower than that in control group(P<0.05).Serum CRP and IgG were positively correlated with SLE complicated with osteonecrosis(r=0.752,0.787,P<0.001).Serum HCT was negatively correlated with SLE complicated with osteonecrosis(r=-0.812,P<0.001).Conclusion The occurrence of osteonecrosis in SLE patients is related to the levels of serum IgG,CRP and HCT.Serum IgG,CRP and HCT are of great significance in differentiating osteonecrosis in SLE patients and its severity,which is worthy of promotion.
