GPC-3和Arg-1在肝细胞性肝癌中的表达及其诊断价值

摘要 目的探讨磷脂酰肌醇蛋白聚糖3(GPC-3)和精氨酸酶1(Arg-1)在肝细胞性肝癌中的诊断价值。方法选取2016年1月至2021年12月接受细针穿刺(FNA)活组织检查的214例患者,其中肝细胞性肝癌110例,肝脏转移癌64例,肝脏非恶性肿瘤40例。用免疫组织化学法检测上述肝组织中GPC-3和Arg-1的表达情况,患者的最终确诊依据病理结果结合临床表现、血清肿瘤标志物及影像学结果。结果GPC-3在肝脏非恶性肿瘤、肝细胞性肝癌、肝脏转移癌中的阳性率分别为2.50%,80.91%,4.69%;Arg-1在肝脏非恶性肿瘤、肝细胞性肝癌、肝脏转移癌中的阳性率分别为97.50%,86.36%、1.56%。GPC-3在肝细胞性肝癌中的阳性表达率高于肝转移癌和肝脏非恶性肿瘤,差异有统计学意义(P<0.05);Arg-1在肝细胞性肝癌中的阳性表达率高于转移癌,差异有统计学意义(P<0.05)。GPC-3在鉴别肝细胞性肝癌与肝脏非恶性肿瘤中的敏感度和特异度分别为80.91%、97.50%;Arg-1在鉴别肝细胞性肝癌与非肝细胞性肝癌中的敏感度和特异度为86.36%、98.44%。结论GPC-3对于肝细胞性肝癌与肝脏非恶性肿瘤、肝细胞性肝癌与肝脏转移癌的鉴别诊断均具有很大价值,Arg-1对于识别肝细胞性肝癌具有较高的敏感度和特异度,同时也是反映肿瘤分化程度的敏感指标。FNA活检行GPC-3、Arg-1免疫组化染色用于肝细胞性肝癌诊断的敏感度和特异度均较高,对于临床工作有很大价值。 Objective To investigate the diagnostic value of glypican-3(GPC-3)and arginase-1(ARG-1)in hepatocellular carcinoma.Methods A total of 214 patients who received fine-needle aspiration(FNA)biopsies from January 2016 to December 2021 were selected,including 110 cases of hepatocellular carcinoma,64 cases of metastatic carcinoma,and 40 cases of benign hepatocellular lesions.Immunohistochemistry was used to detect the expression of GPC-3 and Arg-1 in the above liver tissues.The final diagnosis of the patients was based on pathological results combined with clinical manifestations,serum tumor markers and imaging results.Results The positive rate of GPC-3 in benign hepatocellular lesions,hepatocellular carcinoma and metastatic carcinoma was 2.50%,80.91%and 4.69%.The positive rate of Arg-1 in benign hepatocellular lesions,hepatocellular carcinoma and metastatic carcinoma was 97.50%,86.36%and 1.56%.The positive expression rate of GPC-3 in hepatocellular carcinoma was higher than that in metastatic carcinoma and benign hepatocellular lesions(P<0.05).The positive expression rate of Arg-1 in hepatocellular carcinoma was higher than that in metastatic carcinoma(P<0.05).The sensitivity and specificity of GPC-3 in differentiating hepatocellular carcinoma from benign hepatocellular lesions were 80.91%and 97.50%.The sensitivity and specificity of Arg-1 in diagnosing hepatocellular carcinoma and MC were 86.36%and 98.44%.Conclusion GPC-3 is of great significance in the differential diagnosis of hepatocellular carcinoma and benign hepatocellular lesions,hepatocellular carcinoma and metastatic carcinoma.Arg-1 is a sensitive and specific marker of hepatocellular carcinoma,which has significant advantages in differentiating hepatocellular carcinoma from liver metastases and is also a sensitive indicator of tumor differentiation.GPC-3 and Arg-1 immunohistochemical staining in FNA biopsy for the diagnosis of hepatocellular carcinoma has high sensitivity and specificity,and is of great value for clinical work.
