剖宫产术后再次妊娠孕妇瘢痕妊娠发生的影响因素

Influencing Factors of Scar Pregnancy in Pregnant Women with Second Pregnancy After Cesarean Section

目的探讨剖宫产术后再次妊娠孕妇瘢痕妊娠发生的影响因素。方法收集2019年5月至2021年5月于信阳市妇幼保健院收治的发生瘢痕妊娠的42例剖宫产术后再次妊娠孕妇的临床资料,设为发生组;另收集医院同期未发生瘢痕妊娠的42例剖宫产术后再次妊娠孕妇,设为未发生组。查阅患者基线资料,并记录研究所需资料,将可能的因素纳入,分析剖宫产术后再次妊娠孕妇发生瘢痕妊娠的影响因素。结果发生组患者距离上次剖宫产时间和有胎盘粘连史、人工流产史、合并子宫内膜炎占比与未发生组比较,差异有统计学意义(P<0.05)。经logistic回归分析,结果显示,有胎盘粘连史、有人工流产史、合并子宫内膜炎是剖宫产术后再次妊娠孕妇发生瘢痕妊娠的危险因素(OR>1,P<0.05),距离上次剖宫产时间长是剖宫产术后再次妊娠孕妇发生瘢痕妊娠的保护因素(OR<1,P<0.05)。结论剖宫产术后再次妊娠孕妇瘢痕妊娠发生的影响因素可能为有胎盘粘连史、有人工流产史、合并子宫内膜炎、距离上次剖宫产时间短。

Objective To explore the influencing factors of scar pregnancy in pregnant women with second pregnancy after cesarean section.Methods The clinical data of 42 pregnant women with scar pregnancy after cesarean section treated in Xinyang Maternal and Child Health Care Hospital from May 2019 to May 2021 were collected and set as the occurrence group.In addition,42 pregnant women who did not have scar pregnancy in the hospital in the same period were collected and set as the non occurrence group.The baseline data of patients were consulted,the data required for the study were recorded,the possible factors were included,and the influencing factors of scar pregnancy in pregnant women with second pregnancy after cesarean section were analyzed.Results There were differences in the time from the last cesarean section,the proportion of the history of placental adhesion and induced abortion and endometritis between the occurrence group and the non occurrence group(P<0.05).The results of logistic regression analysis showed that risk factors of scar pregnancy in pregnant women with second pregnancy after cesarean section were placental adhesion,induced abortion and endometritis(OR>1,P<0.05),the long time from the last cesarean section was the protective factor of scar pregnancy in pregnant women with second pregnancy after cesarean section(OR<1,P<0.05).Conclusion The influencing factors of scar pregnancy in pregnant women with second pregnancy after cesarean section may be the history of placental adhesion,induced abortion,endometritis and the short time from the last cesarean section.
