肌骨超声检查在创伤性浅表软组织损伤中的临床应用价值分析

Clinical value of musculoskeletal ultrasound in superficial traumatic soft tissue injury

目的:研究肌骨超声检查在创伤性浅表软组织损伤中的临床应用价值。方法:选取2020年1月—2022年1月兰州市西固区中医院收治的疑似创伤性浅表软组织损伤患者150例,分别采用肌骨超声检查和MRI检查。以手术病理结果作为金标准,分析两种检查方法的检出结果,比较两种方法的诊断效能及对不同病灶的检出情况。结果:以手术病理结果为金标准,肌骨超声检出阳性率为80.67%,与MRI检查的84.67%比较差异不显著(P> 0.05);两种方法的诊断敏感度、特异度、准确度、阳性预测值、阴性预测值相比均无显著差异(P>0.05);两种方法检查肌腱撕裂、韧带撕裂、囊型肿块、实性肿块、神经损伤方面的准确率无显著差异(P> 0.05);肌骨超声检查半月板撕裂的准确率显著低于MRI检查(P<0.05)。结论:在创伤性浅表软组织损伤的诊断中,肌骨超声检查和MRI检查效果相当,但肌骨超声有更广的适用范围和更高的灵活性,同时检查费用较低,患者更容易接受,因此可作为此类疾病的初筛诊断方法,具有较高的临床应用价值。

Objective To study the clinical application value of musculoskeletal ultrasound in traumatic superficial soft tissue injury.Methods A total of 150 patients with suspected superficial traumatic soft tissue injury admitted to Lanzhou Xigu District Traditional Chinese Medicine Hospital from January 2020 to January 2022 were selected.Musculoskeletal ultrasonography and MRI were performed respectively.The pathological results of surgery were used as the gold standard to analyze the detection results of the two methods,and to compare the diagnostic efficiency of the two methods and the detection of different lesions.Results Taking the surgical pathological results as the gold standard,the positive rate of musculoskeletal ultrasound was 80.67%,which was not significantly different from 84.67% of MRI(P>0.05).There was no significant difference in diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value between the two methods(P>0.05).There was no significant difference in the accuracy of tendon tear,ligament tear,cystic mass,solid mass and nerve injury between the two methods(P>0.05).The accuracy of ultrasound examination of meniscus tear was significantly lower than that of MRI examination(P<0.05).Conclusion In the traumatic the diagnosis of superficial soft tissue injuries,musculoskeletal ultrasound and MRI have the same effect,but the musculoskeletal ultrasound has a wider scope and higher flexibility,low examination cost at the same time,patients are more likely to accept,so can be used for early screening of these diseases diagnosis method,has high clinical value.
