摘要
本文探究MR T2 mapping定量参数在肩袖损伤(RCT)患者中水平变化情况及与预后功能转归的关系。选取80例运动致RCT患者为研究对象,均行MRI常规序列扫描和MR T2 mapping扫描,根据RCT分级分为A组(Ⅰ级,n=25)、B组(Ⅱ级,n=39)、C组(Ⅲ级,n=16)。通过对比分析发现,A组冈上肌腱内侧、外侧、中间T_(2)值均低于B组、C组,B组低于C组(P<0.05);冈上肌腱内侧、外侧、中间T_(2)值均与RCT分级呈正相关(P<0.05);RCT患者术后6个月冈上肌腱内侧、外侧、中间T_(2)值及上肢功能障碍评定表(DASH)分值均低于术前,主观肩关节评分(SSV)分值高于术前(P<0.05);RCT患者手术前后冈上肌腱内侧、外侧、中间ΔT_(2)值与ΔSSV、ΔDASH分值呈正相关(P<0.05)。由此可见,MR T2 mapping定量参数与RCT患者的RCT分级呈正相关,且各参数变化值与预后功能转归密切相关。
The changes of MR T2 mapping quantitative parameters in exercise-induced rotator cuff tear(RCT)patients and their relationship with the prognosis and functional outcome were explored in this paper.A total of 80 patients with exercise-induced RCT were selected as the research objects.All of them underwent MRI routine scanning and MR T2 mapping scan,and divided into group A(levelⅠ,n=25),group B(levelⅡ,n=39),and group C(levelⅢ,n=16)according to the RCT classification.Through comparative analysis,it was found that the medial,lateral,and intermediate T_(2) values of the supraspinatus tendon in group A were lower than those in groups B and C,and group B was lower than that of group C(P<0.05);the medial,lateral,and middle T_(2) values of the supraspinatus tendon were all lower than the RCT grading was positively correlated(P<0.05);the medial,lateral,and intermediate T_(2) values of the supraspinatus tendon and the upper extremity Disability of arm shoulder and hand(DASH)scores of the RCT patients were lower than the preoperative,subjective shoulder value(SSV)scores were higher than preoperative(P<0.05);the medial,lateral,and middleΔT_(2) values of supraspinatus tendon before and after surgery were positively correlated withΔSSV andΔDASH scores(P<0.05).It can be seen that the quantitative parameters of MR T2 mapping are positively correlated with the RCT grading of patients with exercise-induced RCT,and the change values of each parameter are closely related to the prognostic function outcome.
