广西住院壮族糖尿病患者代谢管理现状及影响因素分析

Analysis of the current status and influencing factors of metabolic management of hospitalized Zhuang diabetic patients in Guangxi

目的:探讨广西住院壮族糖尿病患者糖化血红蛋白(HbA1c)、血压(BP)和低密度脂蛋白(LDL)(即3B综合指标)的管理现状及其影响因素。方法:于2021年1~12月对广西7家医院6360例住院治疗的壮族糖尿病患者进行调查,分析人口学特征、血糖、血脂等生化指标。结果:调查对象平均年龄(57.6±11.9)岁,平均体重指数(24.2±3.7)kg/m^(2),HbA1c水平为(7.9±2.9)%。HbA1c<7%、BP<18.67/10.67 k Pa、LDL<5.6 mmol/L达标率分别为47.1%、38.1%、31.2%。3B综合指标达标率为4.8%。Logistic回归分析显示,年龄、体重指数、糖尿病病程、使用降糖类药物治疗均为住院壮族糖尿病患者HbA1c、BP和LDL不完全达标的影响因素(均P<0.05)。结论:广西壮族糖尿病患者的HbA1c、BP、LDL达标的比例较低,年龄45~74岁、糖尿病病程≥10年、肥胖和使用降糖类药物是影响3B达标的因素。

Objective:To investigate the current status of management of glycosylated hemoglobin(HbA1c),blood pressure(BP)and low-density lipoprotein(LDL)(i.e.,the 3B composite index)in hospitalized Zhuang diabetic patients in Guangxi and its influencing factors.Methods:A survey of 6,360 Zhuang diabetic patients hospitalized in 7 hospitals in Guangxi were conducted from January to December 2021 to analyze their demographic characteristics,blood glucose,lipids,and other biochemical indices.Results:The mean age of the subjects was(57.6±11.9)years,the mean body mass index was(24.2±3.7)kg/m^(2),and the HbA1c level was(7.9±2.9)%.The goal attainments of HbA1c<7%,BP<18.67/10.67 kPa,and LDL<5.6 mmol/L were 47.1%,38.1%,and31.2%,respectively.The goal attainment of 3B composite index was 4.8%.Logistic regression analysis showed that age,body mass index,duration of diabetes mellitus,and use of glucose-lowering agents were influencing factors for incomplete goal attainments of HbA1c,BP and LDL in Zhuang diabetic patients(all P<0.05).Conclusion:The goal attainments of HbA1c,BP,and LDL in Zhuang diabetic patients in Guangxi were very low,and age 45-74 years,duration of diabetes≥10 years,obesity,and use of glucose-lowering agents were factors influencing 3B goal attainment.
