颈动脉粥样硬化超声参数与冠心病病变程度的相关性分析 被引量:3

The Correlation Analysis of Ultrasonic Parameters of Carotid Atherosclerosis with the Lesion Degree of Coronary Heart Disease

摘要 目的:评价颈动脉粥样硬化超声参数与冠心病病变程度的相关性。方法:选择150例冠心病患者作为研究对象,根据患者冠脉狭窄程度分为单支组、双支组和多支组。根据Gensini评分分为轻度病变组、中度病变组和重度病变组,另外选择同期入院体检的50位健康志愿者作为对照组。检测各组受试者的颈动脉粥样硬化超声参数[颈总动脉硬化度(β)、颈动脉顺应性(AC)、弹性系数(Ep)、脉搏波传导速度(PWVβ)],心功能参数[左室舒张末内径(LVEDD)、左室收缩末内径(LVESD)、左室射血分数(LVEF)]。结果:冠心病患者的β、AC、Ep、PWVβ、LVEDD、LVESD均显著高于对照组(P<0.05),并且随着冠脉病变累及支数及Gensini积分的增加而升高(P<0.05);冠心病患者的LVEF显著低于对照组(P<0.05),并且随着冠脉病变累及支数及Gensini积分的增加而降低(P<0.05);相关性分析显示,颈动脉粥样硬化超声参数与LVEDD、LVESD呈正相关(P<0.05),而与LVEF呈负相关(P<0.05)。结论:颈动脉粥样硬化超声参数及心功能参数与冠心病病变程度具有相关性,在临床预测中具有一定的参考价值。 Objective:To evaluate the correlation between carotid atherosclerotic ultrasound parameters and the degree of coronary heart disease lesions.Methods:150 patients with coronary heart disease admitted to our hospital were included in the study and divided into single-branch,double-branch and multi-branch groups according to the degree of coronary stenosis.And they were divided into mild,moderate and severe lesions according to the Gensini score.Meanwhile,50 healthy volunteers who were admitted to the hospital for routine health checks were recruited as the control group.This study tested the ultrasound parameters of carotid artery atherosclerosis[common carotid artery sclerosis(β),carotid artery compliance(AC),elastic coefficient(Ep),pulse wave conduction velocity(PWVβ)],cardiac function parameters[left ventricular end-diastolic inner diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF)].among all the subjects in each group.Results:The study found that theβ,AC,Ep,PWVβ,LVEDD and LVESD levels of patients with coronary artery disease were all higher than the corresponding levels in the control group(P<0.05).The values increased with the increase in the number of branches involved in coronary artery disease and Gensini score(P<0.05).The LVEF of the patients with coronary artery disease were lower than they were in the control group(P<0.05).The LVEF decreased with the increase in the number of branches involved in coronary artery disease and Gensini score(P<0.05).The correlation analysis indicated that the ultrasound parameter of carotid atherosclerosis has a significant positive relation with the LVEDD,LVESD levels(P<0.05)and a negative relation with the LVEF levels(P<0.05).Conclusion:The ultrasound parameter of carotid atherosclerosis and cardiac function are useful in the clinical prediction of the degree of coronary heart disease.
