基于数据挖掘分析针刺治疗青少年近视选穴规律

Analysis of acupuncture point selection rule in treating juvenile myopia with acupuncture based on data mining

目的 基于数据挖掘技术探析针刺治疗青少年近视的选穴规律。方法 检索建库至2020年12月31日中国知网、重庆维普全文数据库、万方医学网、中国生物医学文献服务系统、PubMed、Cochrance Libarary收录的针刺治疗青少年近视的文献,并筛选符合纳入标准的文献,采用Excel软件建立针刺处方数据库,采用古今医案云平台软件对数据进行腧穴频次、所属经脉、穴位关联度及复杂网络分析。结果(1)一般情况:共纳入文献95篇,腧穴处方共96个,用穴总数为71个,总频次714次,涉及9条经脉。(2)腧穴频次:高频腧穴排在前5位的是瞳子髎(78次,10.92%)、攒竹(63次,8.82%)、四白(55次,7.70%)、睛明(53次,7.42%)、合谷(41次,5.74%)。(3)所属经脉:选穴数目及应用频次排在前5位的是足少阳胆经(168次,23.53%)、足太阳膀胱经(142次,19.89%)、足阳明胃经(111次,15.55%)、手阳明大肠经(41次,5.74%)、手少阳三焦经(30次,4.20%)。(4)腧穴关联度分析:相关性最高的前5位是攒竹-瞳子髎、瞳子髎-四白、四白-攒竹、睛明-瞳子髎、四白-睛明;支持度最高的前5位是攒竹-瞳子髎、瞳子髎-攒竹、睛明-瞳子髎、瞳子髎-睛明、四白-瞳子髎。(5)复杂网络分析:核心处方为四白、风池、攒竹、合谷、光明、丝竹空、太阳、鱼腰及睛明。结论 针刺治疗青少年近视选穴以足三阳经穴为主,常刺激眼周腧穴所在部位的神经,可调节神经功能达到治疗近视的目的,同时应重视局部取穴与远部取穴的联合应用。

OBJECTIVE To analyze the rule of acupuncture point selection in the treatment of juvenile myopia with acupuncture using data mining technology.METHODS The literature on acupuncture treatment of juvenile myopia was retrieved from electronic databases including CNKI Chongqing VIP,Wan Fang,SinoMed,PubMed and Cochrance Libarary from the establishment of these databases to December 31,2020.The literatures that met the inclusion criteria were screened,and the acupuncture prescription database was established using Excel software.The ancient and modern medical record cloud platform was used to analyze the data on acupoint frequency,meridians,acupoint correlation and complex network.RESULTS(1) Genera information:A total of 95 literature were included,involving 96 acupoin prescriptions,71 acupuncture points714 times of acupoint frequency,and nine meridians.(2) Acupoint frequency Thetopfivehigh-frequenc acupuncture points were Tongziliao (GB)(78 times,10.92%),Cuanzhu (GL)(63 times,8.82%)Sibai (ST)(55 times,7.70%),Jingming (BL)(53 times,7.42%) and Hegu (LI)(41 times,5.74%)(3) Meridians:The top five meridians in the number of acupuncture points selected and frequency of application were the gallbladder meridian of foot-Shaoyang (GB)(168 times23.53%),the bladder meridian of foot-Taiyang (BL)(142 times,19.89%),the stomach meridian of foot-Yangming (ST)(111 times,15.55%),the large intestine meridian of hand-Yangming(LI)(41 times,5.74%) and the triple energizer meridian of hand-Shaoyang (TE)(30 times4.20%).(4) Correlation degree analysis of acupoints:The top five combinations of acupoints with the highest correlation were Cuanzhu (GL)-Tongziliao (GB),Tongziliao (GB)-Sibai (ST),Siba(ST)-Cuanzhu (GL),Jingming (BL)-Tongziliao (GB),and Sibai (ST)-Jingming (BL).The top five combinations of acupoints with the highest support were Cuanzhu (GL)-Tongziliao (GB)Tongziliao (GB)-Cuanzhu (GL),Jingming (BL)-Tongziliao (GB),Tongziliao (GB)-Jingming(BL),and Sibai (ST)-Tongziliao (GB).(5) Analysis of complex network of acupoints:The cor prescriptions were Sibai (ST),Fengchi (GB),Cuanzhu (GL),Hegu (LI),Guangming (GB)Sizhukong (TE),Taiyang (EX-HN),Yuyao (EX-HN),and Jingming (BL).CONCLUSIONS Acupuncture is mainly used to treat juvenile myopia with acupuncture points that belonged to the foot three Yang meridians,which stimulated the nerves around the eyes where the acupoints ar located,thus regulating the nerve function to achieve the purpose of treating myopia.At the sam time,the combined application of local acupoints and distal acupoints is quite important.
