左韬从“五脏之气”论治络瘀暴盲

Treatment of ZUO Tao on sudden blindness due to collateral-stasis from the "Qi of five viscera"

视网膜静脉阻塞(RVO)是导致中老年视力障碍的常见瞳神疾病,属于中医络瘀暴盲范畴。左韬教授根据脏腑、气血理论,提出从五脏之气辨治络瘀暴盲。血液的正常运行与五脏之气密切相关,一脏或多脏的脏气功能失常,皆可影响血运而致目络瘀阻,血溢脉外,临床上分为肝失疏泄,气滞血瘀证;脾不统血,血溢络外证;脾失健运,痰瘀互结证;肺失宣肃,水湿停滞证;肺气不足,目络闭塞证;心气不足,目络失养证;肾气亏虚,水湿上犯证。治疗多针对五脏之气从虚或从郁论治,气不足而“虚”者予以益气补虚之法,气不畅而“郁”者予以行气开郁之法。

Retinal vein occlusion(RVO) is a common pupil disease leading to visual impairment in middle-aged and elderly people, it belongs to the category of sudden blindness of Traditional Chinese Medicine. According to the theory of viscera and bowels, Qi and blood,Professor ZUO Tao puts forward that the treatment of sudden blindness due to collateral-stasis from Qi of five viscera. The normal circulation of blood is closely related to Qi of five viscera, thus visceral Qi dysfunction of one or more viscera can affect blood circulation and cause eye collaterals stasis, blood spilled outside of veins.In clinical practice,it can be divided into syndrome of liver failing in free coursing, Qi stagnation and blood stasis;syndrome of spleen failing to control the blood, blood spilled out of veins;syndrome of the spleen failing in transportation,binding of phlegm and stasis;syndrome of the lung failing in purification and down-sending, waterdampness stagnation;syndrome of lung Qi deficiency, eye collaterals stasis;syndrome of heart Qi deficiency, eye collaterals loss of nutrition;syndrome of kidney Qi deficiency, water-dampness invading upward. The treatment mainly targets Qi of five viscera, to rectify deficiency or depression. Specifically, for the cases with deficiency due to lack of Qi, she uses the method of replenishing Qi and tonifying deficiency;For the cases with depression due to unsmoothing Qi flow,she uses the method of regulating Qi and relieving depression.
