复方血栓通联合全视网膜光凝治疗糖尿病黄斑水肿的效果

Effect of compound Xueshuantong combined with panretinal photocoagulation in the treatment of diabetic macular edemal

目的探讨复方血栓通联合全视网膜光凝治疗糖尿病黄斑水肿(DME)的效果。方法选取2018年1月至2020年1月西安大兴医院和延安大学咸阳医院眼科收治的58例(64眼)DME住院患者作为研究对象,按照随机对照法将其分为试验组(29例,32眼)和对照组(29例,32眼)。试验组给予复方血栓通与全视网膜光凝联合治疗,对照组给予全视网膜光凝治疗。比较两组的治疗效果。结果治疗后,试验组的超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、血管内皮生长因子(VEGF)水平均低于对照组(P<0.05)。治疗后,试验组的甘油三酯(TG)、总胆固醇(TCHO)、低密度脂蛋白(LDL)水平均低于对照组,高密度脂蛋白(HDL)水平高于对照组(P<0.05)。治疗后,试验组的最佳矫正视力(BCVA)高于对照组,黄斑中心凹视网膜厚度(CMT)小于对照组(P<0.05)。试验组治疗后的眼水肿时间、出血时间及渗出液吸收时间均短于对照组(P<0.05)。结论复方血栓通联合全视网膜光凝可有效改善DME患者炎症因子水平、血脂指标、视力及黄斑厚度,还可有效提高其术后康复速度。

Objective To investigate the effect of compound Xueshuantong combined with panretinal photocoagulation in the treatment of diabetic macular edema(DME).Methods A total of 58 patients(64 eyes)with DME admitted in the ophthalmology department of Xi'an Daxing Hospital and Xianyang Hospital of Yan'an University from January 2018 to January 2020 were selected as the research objects.According to the randomized control method,the patients were divided into experimental group(29 cases,32 eyes)and control group(29 cases,32 eyes).The experimental group was treated with compound Xueshuantong combined with panretinal photocoagulation,while the control group was treated with panretinal photocoagulation.The therapeutic effects of the two groups were compared.Results After treatment,the levels of hypersensitive C-reactive protein(hs-CRP),interleukin-6(IL-6)and vascular endothelial growth factor(VEGF)in the experimental group were lower than those in the control group(P<0.05).After treatment,the levels of triglyceride(TG),total cholesterol(TCHO)and low density lipoprotein(LDL)in the experimental group were lower than those in the control group,while the level of high density lipoprotein(HDL)was higher than that in the control group(P<0.05).After treatment,the best corrected visual acuity(BCVA)of the experimental group was higher than that of the control group,and the central macular thickness(CMT)was smaller than that of the control group(P<0.05).The eye edema time,bleeding time and exudate absorption time after treatment of the experimental group were shorter than those of the control group(P<0.05).Conclusion The compound Xueshuantong combined with panretinal photocoagulation can effectively ameliorate the inflammatory factors levels,blood lipid indexes,visual and macular thickness of patients with DME,and can also effectively improve the postoperative rehabilitation speed.
