米索前列醇联合马来酸麦角新碱治疗产后出血的效果观察

Effect of Misoprostol Combined with Ergosterone Maleate on Postpartum Hemorrhage

目的分析产后出血患者采用米索前列醇联合马来酸麦角新碱治疗的临床效果。方法选取87例剖宫产术后出血产妇作为研究对象,采用抛硬币法将产妇随机分为研究组(44例)与对照组(43例)。对照组给予米索前列醇片治疗,研究组在对照组基础上加用马来酸麦角新碱治疗。比较两组治疗前后凝血指标、产后出血控制效果、不良反应发生率及临床疗效。结果治疗后,两组纤维蛋白原(FIB)及血红蛋白(Hb)水平均较治疗前明显改善,且研究组FIB、Hb水平优于对照组,差异有统计学意义(P<0.05)。研究组治愈率为95.45%,显著高于对照组治愈率的69.77%,差异有统计学意义(P<0.05);研究组不良反应发生率为11.36%,与对照组的9.30%比较,差异无统计学意义(P>0.05)。研究组产后2 h出血量、产后2~24 h出血量均少于对照组,产后恶露持续时间及住院时间均短于对照组,子宫收缩持续时间长于对照组,差异有统计学意义(P<0.05)。结论米索前列醇联合马来酸麦角新碱治疗剖宫产术后出血产妇疗效显著,可有效控制其产后出血量,维持产妇产后凝血功能,加快康复进程。

Objective To analyze the clinical effect of misoprostol combined with ergostine maleate in the treatment of postpartum hemorrhage.Methods 87 parturients with hemorrhage after cesarean section were selected as the research objects.They were randomly divided into study group(n=44)and control group(n=43)by coin toss method.The control group was treated with misoprostol tablets,and the study group was treated with ergostine maleate on the basis of the control group.The coagulation index before and after treatment,control effect of postpartum hemorrhage,incidence of adverse reactions and clinical efficacy were compared between the two groups.Results After treatment,the levels of fibrinogen(FIB)and hemoglobin(Hb)in the two groups were significantly improved compared with those before treatment,and the levels of FIB and Hb in the study group which were better than those in the control group,and the differences were statistically significant(P<0.05).The cure rate of the study group was 95.45%,which was significantly higher than 69.77%of the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in the study group was 11.36%,which was not significantly different from 9.30%in the control group(P>0.05).The 2 h postpartum blood loss,2~24 h postpartum blood loss,duration of postpartum lochia and length of hospital stay in the study group were significantly lower than those in the control group,and the duration of uterine contractions was significantly longer,and the differences were statistically significant(P<0.05).Conclusion Misoprostol combined with ergonew maleate in the treatment of postpartum hemorrhage after cesarean section is effective,which can effectively control the amount of postpartum bleeding,maintain postpartum coagulation function,and accelerate the process of rehabilitation.
