质子泵抑制剂对伴咽喉反流的难治性鼻窦炎患者术后鼻内镜评分、咽喉症状的改善情况

Effect of Proton Pump Inhibitor on Nasal Endoscopy Score and Improvement of Pharyngeal Symptoms in Patients with Refractory Sinusitis with Laryngopharyngeal Reflux

目的探究质子泵抑制剂对伴咽喉反流的难治性鼻窦炎患者术后鼻内镜评分、咽喉症状的改善情况。方法采取随机数字表法对本院2019年2月至2021年4月收治的合并伴咽喉反流的难治性鼻窦炎患者90例进行分组,各45例,对照组术后给予常规治疗,观察组在对照组基础上增加质子泵抑制剂治疗,对比两组患者术前、术后1个月、术后3个月及术后6个月的Lund-Kennedy鼻内镜评分、Lund-Mackay鼻窦CT评分及反流症状指数量表(RSI)、反流体征评分量表(RFS)评分。结果两组术前Lund-Kennedy鼻内镜评分、Lund-Mackay鼻窦CT评分、RSI、RFS评分相比,差异无统计学意义(P>0.05)。两组术后1个月、术后3个月、术后6个月分别与术前相比Lund-Kennedy鼻内镜评分、Lund-Mackay鼻窦CT评分、RSI、RFS评分均较低,术后3个月与术后1个月相比术前相比Lund-Kennedy鼻内镜评分、Lund-Mackay鼻窦CT评分、RSI、RFS评分均较低,术后6个月与术后3个月相比术前相比Lund-Kennedy鼻内镜评分、Lund-Mackay鼻窦CT评分、RSI、RFS评分均较低,差异有统计学意义(P<0.05)。结论将质子泵抑制剂应用到伴咽喉反流的难治性鼻窦炎患者术后可促进患者鼻部症状的改善,改善咽部反流情况,总体预后表现更好。

Objective To explore the effect of proton pump inhibitors on the nasal endoscopic score and the improvement of pharyngeal symptoms in patients with refractory sinusitis with pharyngolaryngeal reflux after surgery.Methods Ninety patients with refractory nasosinusitis complicated with laryngopharyngeal reflux admitted to our hospital from February 2019 to April 2021 were randomly divided into 45 cases each.The control group was given routine treatment after surgery.The observation group was added proton pump inhibitor treatment on the basis of the control group.The Lund-Kennedy nasal endoscopy scores Lund-Mackay nasal sinus CT score and reflux symptom index(RSI),reflux symptom scale(RFS).Results There was no significant difference between the two groups in preoperative Lund-Kennedy nasal endoscopy score,Lund-Mackay sinus CT score,RSI,RFS score(P>0.05).The Lund-Kennedy nasal endoscopy score,Lund-Mackay nasal sinus CT score,RSI and RFS score of the two groups were lower 1 month,3 months and 6 months after surgery than before surgery,respectively.The Lund-Kennedy nasal endoscopy score,Lund-Mackay nasal sinus CT score,RSI and RFS score of the two groups were lower 3 months after surgery than 1 month after surgery.The Lund-Nasal endoscopy score,Lund-Mackay nasal sinus CT score,RSI and RFS score of the two groups were lower 6 months after surgery than 3 months after surgery Lund-Mackay sinus CT score,RSI,RFS score were lower,the differences were statistically significant(P<0.05).Conclusion The application of proton pump inhibitor to the patients with refractory sinusitis with pharyngolaryngeal reflux can improve the nasal symptoms and pharyngeal reflux,and the overall prognosis is better.
