穴位按摩辅助甘精胰岛素对老年糖尿病患者血糖及Netrin-1、Irisin水平的影响

Effect of acupoint massage-assisted insulin glargine on blood glucose index and Netrin-1 and Irisin levels in elderly diabetic patients

目的探讨穴位按摩辅助甘精胰岛素对老年糖尿病患者血糖及轴突导向因子-1(Netrin-1)、鸢尾素(Irisin)水平的影响。方法选取2019年6月至2021年6月绍兴文理学院附属医院收治的老年糖尿病患者165例,采用随机数字表法分为对照组83例、联合组82例。对照组予甘精胰岛素治疗,联合组予穴位按摩辅助甘精胰岛素治疗。检测两组患者的血糖相关指标、胰岛素抵抗水平变化,通过黄嘌呤氧化酶法测定超氧化物歧化酶(superoxide dismutase,SOD)水平、硫代巴比妥酸法检测丙二醛(malondialdehyde,MDA)水平;酶联免疫吸附实验法检测白介素细胞-17(interleukin 17,IL-17)、白介素细胞-1β(interleukin-1β,IL-1β)、Irisin水平;全自动生化分析仪检测两组患者的血脂指标;RT-PCR法测定Netrin-1相对表达量。结果治疗后,与对照组比较,联合组患者糖化血红蛋白(hemoglobin Alc,HbAlc)、空腹血糖(fasting blood glucose,FBG)、餐后2h血糖(2-hour postprandial blood glucose,2hPG)、胰岛素抗生指数(insulin life resistance index,HOMA-IR)、MDA、IL-17、IL-1β、总胆固醇(serum total cholesterol,TC)、三酰甘油(triacylglyceride,TG)、低密度脂蛋白胆固醇(high density lipoprotein cholesterol,LDL-C)水平、Netrin-1表达水平明显降低(P<0.05),胰岛β细胞功能指数(index ofβ-cell function in HOMA,HOMA-β)、SOD、高密度脂蛋白胆固醇(high level data link control,HDL-C)水平、Irisin表达升高(P<0.05)。结论使用穴位按摩辅助甘精胰岛素对老年糖尿病患者进行治疗,患者血糖水平、胰岛素抵抗明显缓解,氧化应激、炎性反应明显减轻,血脂水平得到显著改善,Netrin-1、Irisin表达受到明显调控,治疗效果更显著。

Objective To explore the effect of acupressure massage-assisted insulin glargine on blood glucose index and neural axonal guidance factor 1(Netrin-1)and irisin(Irisin)levels in elderly diabetic patients.Methods A total of 165 elderly diabetes patients admitted to Shaoxing Wenli University Hospital from June 2019 to June 2021 were selected,included 83 patients in the control group and 82 patients in the combination group.The control group was treated with insulin glargine,and the combined group gave acupuncture point massage-assisted insulin glargine for treatment.Changes in blood glucose-related indicators and insulin resistance levels in the two groups,superoxide dismutase was measured by the xanthine oxidase method superoxide dismutase(SOD)horizontal;The malondialdehyde was detected by thiobarbituric acid method malondialdehyde(MDA)horizontal;Enzyme-linked immunosorbent assay for detecting interleukin-17(IL-17),interleukinβ(IL-1β),Irisin level;Automatic biochemical analyzer to detect the blood lipid level;Relative expression level of Netrin-1 was determined by RT-PCR.The treatment situation between the two groups was compared.Results Post-treatment,compared with the control group,in the combination group,hemoglobin A1c(HbA1c),fasting blood glucose(FBG),two-hour postprandial blood glucose(2hPG),insulin resistance index(HOMA-IR),MDA,IL-17,IL-1β,total cholesterol(TC),triacylglycerol(TG),high density lipoprotein cholesterol(LDL-C)level,Netrin-1 expression was decreased(P<0.05),Islet cell function index(HOMA-),SOD,HD density lipoprotein cholesterol(HDL-C)level and Irisin expression were increased(P<0.05).Conclusion Acupoint massage-assisted insulin glargine is used to treat elderly patients with diabetes.The blood glucose level and insulin resistance was significantly relieved,the oxidative stress and inflammatory response was significantly reduced,the blood lipid level is significantly improved,the expression of Netrin-1 and Irisin were significantly regulated,and the treatment effect is remarkable.
