雷火灸联合乌灵胶囊对气虚血瘀证卒中后疲劳的干预研究

Study on the intervention of thunder fire moxibustion combined with Wuling Jiaonang on poststroke fatigue of Qi deficiency and blood stasis syndrome

目的观察雷火灸联合乌灵胶囊治疗气虚血瘀证卒中后疲劳(poststroke fatigue,PSF)的疗效。方法选取2021年8月至2022年3月于广州市中西医结合医院住院的PSF患者200例,按照随机数字表法将患者分为空白对照组、乌灵胶囊组、雷火灸组和联合组(乌灵胶囊+雷火灸),每组各50例。比较治疗前后4组患者的疲劳严重程度量表(fatigue severity scale,FSS)、汉密尔顿抑郁量表(Hamilton depression scale,HAMD)、美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)及改良Rankin量表(modified Rankin scale,MRS)评分。结果治疗3个月,4组患者的FSS、HAMD、NIHSS评分均显著低于本组治疗前(P<0.05),雷火灸组、乌灵胶囊组和联合组患者的MRS评分均显著低于本组治疗前(P<0.05);治疗3个月,联合组患者的FSS、HAMD、NIHSS及MRS评分均显著低于其他3组(P<0.05)。结论雷火灸联合乌灵胶囊对气虚血瘀证PSF有显著疗效,可改善脑卒中后患者的疲劳症状,具有临床推广价值。

Objective To study the effect of thunder fire moxibustion(TFM)combined with Wuling Jiaonang on poststroke fatigue(PSF)of Qi deficiency and blood stasis syndrome.Methods A total of 200 PSF patients hospitalized in Guangzhou Hospital of Integrated Traditional and Western Medicine from August 2021 to March 2022 were selected and divided into blank control group,Wuling Jiaonang group,TFM group and combination group(Wuling Jiaonang+TFM)according to the random number table method,with 50 cases in each group.The fatigue severity scale(FSS),Hamilton depression acale(HAMD),National Institute of Health stroke scale(NIHSS)and modified Rankin scale(MRS)were compared before and after treatment.Results Three months after treatment,the FSS,HAMD and NIHSS scores of the four groups were significantly lower than those before treatment(P<0.05),and the MRS scores of patients in TFM group,Wuling Jiaonang group and combination group were significantly lower than those before treatment(P<0.05).Three months after treatment,the FSS,HAMD,NIHSS and MRS scores of the combined group were significantly lower than those of the other three groups(P<0.05).Conclusion TFM combined with Wuling Jiaonang has a significant effect on PSF of Qi deficiency and blood stasis syndrome,and can improve the fatigue symptoms of poststroke patients,which has clinical promotion value.
