磁共振弥散加权成像在前列腺癌术前淋巴结转移中的预测价值 被引量:3

The prediction value of MR diffusion-weighted image in the diagnosis of lymph node metastasis of prostate cancer

摘要 目的 探讨磁共振弥散加权成像(DWI)在前列腺癌术前淋巴结状态评估中的预测价值。方法 回顾性分析我院经病理证实的40例前列腺癌患者的临床、病理及影像学资料,所有患者均接受前列腺根治术及淋巴结清扫术,在术前行常规核磁共振(MRI)及DWI检查。两名高年资腹部影像学诊断医师分别测量前列腺癌病灶的表观弥散系数(ADC),在前列腺癌术前淋巴结状态评估中的预测价值其诊断前列腺癌术前淋巴结转移的效能。结果 两名医师测量前列腺癌病灶平均ADC值的一致性较好。40例患者中29例为淋巴结转移,11例无淋巴结转移。其中淋巴结转移阳性组和阴性组的平均ADC值分别为(0.578±0.106)×10^(-3) mm^(2)s、(0.821±0.172)×10^(-3) mm^(2)s,两组间差异有统计学意义(P<0.05)。当以平均ADC值0.74×10^(-3) mm^(2)s作为鉴别术前有无淋巴结转移时的阈值,其ROC曲线下面积为0.867,敏感性为94.7%,特异性为71.4%。结论 肿瘤的平均ADC值对于前列腺癌淋巴结转移具有较好的预测价值。
